[目的]研究胰岛素抵抗(IR)与心血管代谢危险因素的相关性.[方法]于2008年8月至2009年8月在延边朝鲜族自治州安图县内采用简单随机抽取方法抽取1 235名常住居民,对其进行健康体格检查和生物化学指标检测.用放射免疫法检测空腹胰岛素(FINS),采用稳态模式评估法估测人体IR水平.血脂及血糖指标均用全自动生物化学分析仪进行测定.[结果]有IR、高血糖、高血压、血脂异常、整体型肥胖及腹型肥胖者的比例分别为25.0%,14.4%,37.5%,23.8%,34.5%,61.3%.简单相关分析及调整民族、性别及年龄因素的影响后偏相关分析结果均显示,SBP,DBP,TC,TG,BMI,WHtR,FBG,FINS均与IR相关(P<0.01).具有2种以上代谢异常者比例随着IR水平增高而上升,且具有2种、3种及4种代谢异常者比例也随着IR水平增高而上升(P<0.01).[结论]延边地区一般人群中IR和常见心血管代谢危险因素的检出率均较高,而且IR者伴发常见代谢异常的现状普遍存在;IR水平与不同代谢异常聚集者的比例呈正相关.
[Objective] To study the correlation between insulin resistance (IR) and risk factors of cardiovascular metabolism. [Methods] From August 2008 to August 2009, a random sampling method was used in Antu County, Yanbian Autonomous Prefecture, Resident, to carry out physical examination and biochemical indicators of detection of fasting insulin (FINS) by radioimmunoassay, the use of steady-state model to assess the level of human IR.Serum lipid and blood glucose indicators were used automatic biochemical analyzer [Results] The rates of IR, hyperglycemia, hypertension, dyslipidemia, general obesity and abdominal obesity were 25.0%, 14.4%, 37.5%, 23.8%, 34.5% and 61.3% respectively. After analyzing and adjusting the influence of ethnicity, gender and age, post-correlation analysis showed that SBP, DBP, TC, TG, BMI, WHtR, FBG and FINS were all related to IR (P <0.01) (P <0.01). [Conclusion] The proportion of IR and common heart in the general population in Yanbian area increased with the increase of IR level, and the proportion of those with two kinds, three kinds and four kinds of metabolic abnormalities also increased with the increase of IR level The detection rate of vascular metabolic risk factors are high, and IR associated with common The prevalence of metabolic abnormalities prevails; the level of IR is positively correlated with the proportion of different metabolic abnormalities.
