目的本研究分析成人Ph染色体阳性急性淋巴细胞白血病(Ph+ALL)髓系抗原表达情况及临床意义。方法采用流式细胞术检测93例初诊的ALL患者免疫表型,其中Ph+ALL 28例,Ph-ALL 65例,并分析其髓系抗原表达和临床意义。结果 Ph+ALL免疫表型均为B细胞,髓系抗原表达明显高于Ph-ALL组(P<0.05),其CD13、CD33的表达明显高于Ph-ALL组(P<0.05),髓系表达的抗原主要为CD13和CD33。Ph+ALL患者CD34的表达明显高于Ph-ALL组(P<0.05)。Ph+ALL组首疗程完全缓解率明显低于Ph-ALL组(P<0.05)。结论Ph+ALL患者CD13、CD33、CD34表达增高,可能与其诱导化疗首疗程完全缓解率低有关,提示未来可用于微小残留病灶(MRD)的监测,同时为潜在的靶向治疗提供理论基础。
Objective This study analyzed the expression and clinical significance of myeloid antigens in adult Ph chromosome positive acute lymphoblastic leukemia (Ph + ALL). Methods Flow cytometry was used to detect the phenotype of 93 newly diagnosed ALL patients, including Ph + ALL in 28 cases and Ph-ALL in 65 cases. The myeloid antigen expression and clinical significance were analyzed. Results The immunophenotypes of Ph + ALL were all B cells. The myeloid antigen expression in Ph + ALL group was significantly higher than that in Ph-ALL group (P <0.05). The expression of CD13 and CD33 in Ph + ALL group was significantly higher than that in Ph-ALL group The expressed antigens are mainly CD13 and CD33. The expression of CD34 in Ph + ALL patients was significantly higher than that in Ph-ALL patients (P <0.05). The complete remission rate of Ph + ALL group was significantly lower than Ph-ALL group (P <0.05). Conclusion The increased expression of CD13, CD33 and CD34 in patients with Ph + ALL may be related to the low complete remission rate of the first course of chemotherapy-induced chemotherapy in Ph + ALL patients, suggesting that it may be used for the monitoring of minimal residual disease (MRD) in the future and provide a theoretical basis for potential targeted therapy.
