目的:探讨超声在终末期肾病患者人工自体动静脉内瘘监测中的应用价值。方法:采用超声对41例终末期肾病患者人工自体动静脉内瘘进行系列二维形态学及血流动力学扫查,主要监测内瘘瘘口处、桡动脉段及头静脉段的内径、峰值流速,观察动静脉内瘘并发症的超声图像表现,必要时测量其血流动力学指标。结果:41例终末期肾病患者中,25例动静脉内瘘结构正常,血流通畅,到达临床血液透析要求;16例出现并发症,其中血栓形成8例,头静脉瘤样扩张4例,血肿形成2例,动静脉内瘘狭窄1例,盗血综合症1例。结论:应用超声对自体动静脉内瘘进行监测,能及时发现并发症,可为临床采取适当治疗措施提供有价值的信息。
Objective: To investigate the value of ultrasound in the monitoring of artificial autologous arteriovenous fistula in patients with end-stage renal disease. Complications were found in 16 patients, of which 8 were thrombosis, 4 were cephalic aneurysm, 4 were hematomas The formation of 2 cases, 1 case of arteriovenous fistula stenosis, steal syndrome in 1 case. Conclusion: Ultrasound can monitor the fistula of autologous arteriovenous fistula and detect the complication in time, which can provide valuable information for clinical treatment.
