目的:了解上海市金山农村癫 患病率、药物治疗现状、癫 患者的就业、教育、家庭和婚姻等社会心理状况。方法:于2000年在上海市金山朱泾镇采用分层整群抽样原则完成5万人群的癫 病普查工作。结果:癫 粗患病率为3.11‰,标化患病率为3.62‰,其中男性3.65‰,女性2.50‰;农村患病率为3.70‰,明显高于城镇的2.45‰;年龄分组患病率分布有两个高峰,第一高峰在10~30岁,第二高峰在60岁年龄组;与正常人群相比,男性患者的文化程度低、无职业率高和未婚率(包括丧偶和离婚)高,而女性无明显差异;原发性癫 (通过调查未了解到病因的癫 )占82.1%,其中有家族史者占18.5%。继发性癫 占17.9%。主要病因为:脑膜炎或脑炎37.0%,颅脑外伤18.5%,精神疾病14.8%,脑血管疾病11.1%,颅内肿瘤11.1%,高血压脑病7.4%。全身强直阵挛性发作和部分发作之比为1.8:1;接受治疗者占66.5%,发作明显控制者占29.3%。结论:上海市金山区居民癫 患病率在国内处于中等偏低水平。部分癫 患者在工作、学习及婚姻等方面受到了影响。
Objective: To understand the prevalence of epilepsy in rural Jinshan in Shanghai, the current status of medical treatment, employment of epilepsy, education, family and marriage and other social psychological conditions. Methods: In 2000, Shanghai Jinshan Zhu Jing town stratified cluster sampling principle to complete the 50,000 population epilepsy survey. Results: The prevalence of epilepsy was 3.11 ‰, the standardized prevalence was 3.62 ‰, of which 3.65 ‰ for males and 2.50 ‰ for females; the prevalence in rural areas was 3.70 ‰, which was significantly higher than 2.45 ‰ in urban areas. There are two peaks in the distribution of the age group. The first peak is between 10 and 30 years old and the second peak is in the 60-year-old group. Compared with the normal population, the male patients have a low educational level, High occupational rate and unmarried rate (including widowed and divorced) were high, but no significant difference between women; primary epilepsy (by investigation did not know the etiology of the epilepsy) accounted for 82.1%, of which family history accounted for 18.5% . Secondary epilepsy accounted for 17.9%. The main causes are: meningitis or encephalitis 37.0%, traumatic brain injury 18.5%, mental illness 14.8%, cerebrovascular disease 11.1%, intracranial tumors 11.1%, hypertensive encephalopathy 7. 4%. The ratio of generalized tonic-clonic seizures to partial seizures was 1.8: 1, 66.5% were treated, and 29.3% had significant seizures. Conclusion: The prevalence of epilepsy among residents in Jinshan District of Shanghai is at a moderately low level in China. Some epilepsy patients have been affected by work, study and marriage.
