目的探讨血清抵抗素、肿瘤坏死因子α(TNF-α)与2型糖尿病胰岛素抵抗的相关性。方法2003-05~2004-01对福建医科大学附属第二医院的42例2型糖尿病患者及40名健康对照者,检测腰围、空腹血糖(FBG)、胰岛素、抵抗素、TNF-α等指标。结果糖尿病组血清抵抗素、TNF-α及Homa-胰岛素抵抗指数(Homa-IR)水平显著高于对照组[(11·76±2·72)对(8·65±3·00)μg/L,P<0·01]、[(0·29±0·05)对(0·19±0·04)μg/L,P<0·01]及[(4·17±1·65)对(2·98±1·04),P<0·01]。血清胰岛素水平前者显著低于后者[(8·91±4·72)对(12·44±4·53)mU/L,P<0·01]。血清抵抗素水平与Homa-IR、FBG及腰围具有相关性(均P<0·05),与TNF-α水平无相关性。血清TNF-α水平与Homa-IR显著呈正相关(P<0·01)。结论血清抵抗素与TNF-α参与2型糖尿病胰岛素抵抗的发生。
Objective To investigate the relationship between serum resistin, tumor necrosis factor-α (TNF-α) and insulin resistance in type 2 diabetic patients. Methods From May 2003 to January 2004, 42 patients with type 2 diabetes mellitus and 40 healthy controls in the Second Affiliated Hospital of Fujian Medical University were enrolled in the study. The indexes of waist circumference, fasting blood glucose (FBG), insulin, resistin and TNF-α were detected. Results The levels of serum resistin, TNF-α and Homa-IR in diabetic group were significantly higher than those in control group [(11 · 76 ± 2 · 72) vs (8 · 65 ± 3 · 00) μg / L (0 · 19 ± 0 · 04) μg / L, P <0 · 01] and [(4 · 17 ± 1 · 65) vs (2 · 98 ± 1 · 04), P <0 · 01]. The former of serum insulin level was significantly lower than the latter [(8.91 ± 4.72) vs (12.44 ± 4.53) mU / L, P <0.01]. Serum resistin levels were correlated with Homa-IR, FBG and waist circumference (all P <0.05), but not with TNF-α levels. Serum TNF-α level was positively correlated with Homa-IR (P <0.01). Conclusions Serum resistin and TNF-α are involved in the development of insulin resistance in type 2 diabetes mellitus.
