目的了解上海市崇明地区8~12岁儿童尿碘水平,推测崇明地区居民碘营养状况,为进一步调整崇明碘盐政策提供理论和指导依据。方法采用整群、分层、随机抽样相结合的方法,收集崇明东西南北中五个方位的5所小学中三~五年级各一个班级的8~12岁学生共725份尿样,采用武汉众生生化技术有限公司生产的尿碘快速定量检测试剂盒(热消解)进行尿碘检测,采用曲线化为直线的回归方程法计算尿碘含量,采用触诊法检查甲状腺大小,并进行分析。结果 725名儿童尿碘水平为(229.5±108.1)μg/L(29.2~660.6μg/L),中位数为217.7μg/L,其中尿碘<100μg/L 59人占8.2%,100~199μg/L 252人占35%,200~299μg/L 255人占35.2%,>300μg/L 154人占21.4%。触诊检查发现甲状腺Ⅰ度肿大11人,Ⅱ度肿大10人,甲肿率为2.9%。结论上海市崇明地区现阶段儿童碘摄入量基本可以满足机体的生理需要,不需要采用特别补碘措施。崇明东部沿海地区儿童尿碘水平明显高于其他区域。对尿碘含量偏高儿童,应指导其合理调整饮食,定期监测尿碘。
Objective To understand the level of urinary iodine in children aged 8 ~ 12 years in Chongming district of Shanghai, and to estimate the iodine nutrition status of residents in Chongming district, so as to provide theoretical and guidance basis for further adjusting the policy of salt iodization in Chongming district. Methods A total of 725 urinary samples from 8 to 12-year-old students in each of the three grades to the fifth grade in five primary schools in five places of Chongming, east, west, north and south of Chongming were collected by means of cluster analysis, stratification and random sampling. The urinary iodine test was performed using the urine iodine rapid quantitative test kit (thermal digestion) produced by Biochemical Technology Co., Ltd. The urine iodine content was calculated by the linear regression equation method. The thyroid size was examined by palpation method and analyzed. Results The urinary iodine level in 725 children was (229.5 ± 108.1) μg / L (29.2 ~ 660.6 μg / L) with a median of 217.7 μg / L, of which, urinary iodine <100 μg / / L 252 accounted for 35%, 200 ~ 299μg / L 255 accounted for 35.2%,> 300μg / L 154 people accounted for 21.4%. Palpation examination revealed thyroid Ⅰ degree enlargement of 11 people, Ⅱ degree enlargement of 10 people, the rate of 2.9%. Conclusion The iodine intake of children in Chongming district of Shanghai at this stage can basically meet the physiological needs of the body without the need of special iodine supplement measures. Urinary iodine levels in children in the eastern coastal areas of Chongming were significantly higher than those in other areas. Children with high urinary iodine content, should guide the rational adjustment of diet, regular monitoring of urinary iodine.
