目的研究2型糖尿病(T2DM)一级亲属的代谢状态以及与炎症因子hs-CRP的相关性。方法将入选患者分成4组,2型糖尿病(T2DM)组41例,糖尿病前期(PDM)组38例,T2DM患者正常糖耐量一级亲属(FDR)组46例,正常对照(NC)组48例。分别测定血清hs-CRP及相关代谢指标。结果在校正年龄、BMI、腰围后,FPG、2hPG、HbA1c、TG、LDL-C和hs-CRP在NC组、FDR组、PDM组和T2DM组均呈增高趋势,而HOMA-β和HDL-C呈降低趋势(P<0.05)。HOMA-IR和FINS在NC组、FDR组和PDM组均呈增高趋势(P<0.05),而在PDM组和T2DM组差异无统计学意义(P>0.05)。hs-CRP与BMI、FPG、2hPG、HOMA-IR及TG呈正相关(P<0.01),与HDL-C呈负相关(P<0.01)。结论 T2DM患者一级亲属在糖耐量正常时就已经出现胰岛素抵抗及脂代谢紊乱,慢性炎症与高胰岛素血症、血脂紊乱及肥胖有关。
Objective To investigate the metabolic status of first-degree relatives of type 2 diabetes mellitus (T2DM) and its relationship with hs-CRP. Methods The patients were divided into four groups: 41 cases in type 2 diabetes mellitus (T2DM), 38 cases in pre-diabetes (PDM) group, 46 cases in T2DM normal glucose tolerance first-degree relatives (FDR) group and 48 cases in normal control group . Serum hs-CRP and related metabolic parameters were measured. Results After adjusting for age, BMI and waist circumference, the levels of FPG, 2hPG, HbA1c, TG, LDL-C and hs-CRP in NC group, FDR group, PDM group and T2DM group all increased while HOMA-β and HDL-C Showed a decreasing trend (P <0.05). HOMA-IR and FINS showed an increasing trend in NC group, FDR group and PDM group (P <0.05), but there was no significant difference between PDM group and T2DM group (P> 0.05). There was a positive correlation between hs-CRP and BMI, FPG, 2hPG, HOMA-IR and TG (P <0.01), but negatively correlated with HDL-C (P <0.01). Conclusions First-degree relatives of T2DM patients have insulin resistance and lipid metabolism disorders when their glucose tolerance is normal. Chronic inflammation is associated with hyperinsulinemia, dyslipidemia and obesity.
