目的了解鲍曼不动杆菌(Ab)在呼吸道感染儿童患者中的分布及其对常用抗生素耐药情况。方法回顾性分析湖南省儿童医院2008年1月-2009年1月从呼吸道感染儿童痰、咽拭子或肺泡灌洗液等标本中分离出的Ab分布特点及耐药情况。结果共检出鲍曼不动杆菌414株,ICU病人检出率最高,占41.8%(173株),其次为呼吸内科33.1%(137株)。5-8月检出株数均大于50,高于其他月份,Ab对哌拉西林、头孢替坦、头孢哌酮、头孢唑啉、氨苄西林耐药率均≥90%,对头孢曲松耐药率为75.8%,对氨苄西林/舒巴坦、哌拉西林/他唑巴坦耐药率均≥20%,对亚胺培南、美洛培南耐药率低,尚未发现耐美洛培南菌株。结论儿童呼吸道感染Ab及耐药形势严峻,应高度重视重点病区、重点时段感染流行的监测及消毒工作,结合药敏,合理使用抗生素。
Objective To understand the distribution of Acinetobacter baumannii (Ab) in children with respiratory tract infections and its resistance to commonly used antibiotics. Methods The distribution characteristics and drug resistance of Ab isolated from specimens of sputum, throat swab or alveolar lavage of children with respiratory tract infection in Hunan Children’s Hospital from January 2008 to January 2009 were retrospectively analyzed. Results A total of 414 strains of Acinetobacter baumannii were detected. The highest detection rate was found in ICU patients, accounting for 41.8% (173 strains), followed by 33.1% (137 strains) in respiratory medicine. The number of strains detected in May-August was more than 50, higher than that in other months. Ab was resistant to piperacillin, cefotetan, cefoperazone, cefazolin and ampicillin, all of which were resistant to ceftriaxone Rate of 75.8%, ampicillin / sulbactam, piperacillin / tazobactam resistance rate were ≥ 20%, imipenem, meropenem resistance rate is low, no methioperoxide has been found South strain. Conclusion Ab and drug resistance of children with respiratory infection are serious. We should attach great importance to monitoring and disinfection of key wards and key period of infection, combined with drug sensitivity and rational use of antibiotics.
