目的 评价经瞳孔温热疗法 (transpupillary thermotherapy,TTT)治疗渗出型老年性黄斑变性 (age- related macular degeneration,AMD)的临床疗效。 方法 回顾分析 6 2例确诊为 AMD的患者的6 2只患眼 TTT治疗的临床资料。6 2例 AMD患眼中 ,行荧光素眼底血管造影 (fundus fluorescein angiogra-phy,FFA)检查者 5 8例 ,吲哚青绿血管造影 (indocyanine green angiography ,ICGA)检查者 4 2例 ,光相干断层成像术 (optic coherence tomography,OCT)检查者 5 6例。激光治疗机波长为 810 nm,光斑 0 .5~3.0 mm ,能量 6 0~ 4 0 0 m W,照射时间 6 0 s。 6 2例患者治疗后随访观察 1~ 10个月 ,平均随访观察时间 4 .8个月 ,分析末次随访视力与治疗前视力以及 OCT复查资料。 结果 视力不变者 4 3例 ,占 6 9.4 % ;进步者15例 ,占 2 4 .2 % ;下降者 4例 ,占 6 .5 %。 5 1例 OCT复查者中 ,黄斑水肿不变者 2 9例 ,占 5 6 .96 % ;好转者18例 ,占 38.3% ;恶化者 4例 ,占 7.8%。视力改善与 OCT黄斑水肿的形态改善相一致者 38只眼 ,占 OCT检查者的 74 .5 % ;视力改善滞后于黄斑水肿形态改善者 13只眼 ,占 2 5 .5 %。再次行 TTT治疗者 18只眼 ,占接受 TTT治疗者的 2 9.0 %。治疗随访期间无明显治疗副作用。 结论 TTT治?
Objective To evaluate the clinical efficacy of transpupillary thermotherapy (TTT) in the treatment of age-related macular degeneration (AMD). Methods The clinical data of 62 eyes with TTT treated in 62 patients diagnosed as AMD were retrospectively analyzed. Fifty-eight cases of fundus fluorescein angiography (fundus fluorescein angiogra-phy, FFA), 42 cases of indocyanine green angiography (ICGA) were examined by light microscopy Sixty-six patients underwent optic coherence tomography (OCT). The wavelength of the laser treatment machine is 810 nm, the spot size is 0.5-3.0 mm, the energy is 60-400 mW and the irradiation time is 60s. Sixty-two patients were followed up for 1 to 10 months after treatment. The mean follow-up time was 4.8 months. The final follow-up vision, pre-treatment visual acuity and OCT were analyzed. The results of visual acuity were 43 cases, accounting for 6 9.4%; progressive in 15 cases, accounting for 24.2%; decreased in 4 cases, accounting for 6.5%. Of the 51 OCT patients, 29 were persistent with macular edema, accounting for 56.96%; 18 were improved, accounting for 38.3%; and 4 were exacerbated, accounting for 7.8%. Visual acuity was improved in 38 eyes, accounting for 74.5% of OCT examiners. Visual acuity was delayed in 13 eyes (25.5%) with macular edema. Twenty-eight eyes of TTT were treated again, accounting for 29.0% of those receiving TTT. No significant side effects during treatment follow-up. Conclusion TTT treatment?
