目的:通过调查住院患者对出院后健康管理的需求情况,为制定患者出院后健康管理措施提供参考依据。方法:参考已有文献,自设住院患者对出院后健康管理需求情况的调查问卷,随机选取800例即将出院的患者为研究对象,进行问卷调查。结果:485例患者渴望得到疾病相关知识及康复指导的健康管理知识,前3位分别是健康指导、家庭护理指导、相关检查准备及注意事项。在健康管理方式上,398例患者希望得到定期电话随访,297例患者希望获得定期的短信提醒。其中195例患者需要为期1个月的定期电话随访健康管理服务,242例患者需要为期1个月的定期短信提醒健康管理服务。结论:患者对出院后的健康管理知识需求较强烈,医院需要给患者提供多种形式的、个体化的健康管理服务,满足患者对院后健康管理的需求。
OBJECTIVE: To provide a reference for formulating the health management measures after hospital discharge by investigating the demand of hospitalized patients for post-discharge health management. Methods: According to the existing literatures, we set up the questionnaire of inpatients’ demand for health management after discharge, and randomly selected 800 patients who will be discharged as the research subjects and conducted a questionnaire survey. Results: 485 patients were eager to get the disease-related knowledge and rehabilitation guidance of health management knowledge, the first three were health guidance, home care guidance, related inspection preparation and precautions. In terms of health management, 398 patients would like regular follow-up and 297 patients would like regular SMS reminders. Of these, 195 patients require a one-month regular telephone follow-up of health management services and 242 patients require a one-month regular SMS reminder of health management services. Conclusion: The patients have strong demand for health management knowledge after discharge. The hospital needs to provide patients with various forms of individualized health management services to meet the needs of patients for post-natal health management.
