背景:苦瓜属植物提取物有降糖效果是已被证实的,而其是否有调脂作用还需进一步研究。目的:评价苦瓜降糖胶囊对老年2型糖尿病的降糖及调脂作用。设计:前后对照试验。地点、对象和方法:研究对象为2003-03收集的35例男性2型糖尿病患者,年龄70~86岁,平均(77±5)岁。纳入研究对象均为军队干休所离退休干部;既往均明确诊断为2型糖尿病,即符合1985年世界卫生组织2型糖尿病诊断标准;所有患者1个月未用过胰岛素及口服降糖药物。排除患有其他严重的躯体性疾病;有精神疾病或精神疾病家族史;有严重智力或认知障碍患者。对35例患者投入的药物为本院制备的苦瓜降糖胶囊。每例患者均在疗程前后立即检测空腹血糖、餐后2h血糖、糖化血红蛋白和血脂。血糖测定为酶试剂法。糖化血红蛋白采用微柱法。总胆固醇、三酰甘油、高密度脂蛋白胆固醇均用酶法。主要观察指标:用药前后的血糖、血脂的变化。结果:采用苦瓜降糖胶囊治疗后患者空腹血糖,餐后2h血糖及糖化血红蛋白犤(5.02±1.67),(9.70±3.44)mmol/L,(5.79±1.18)%犦均有较治疗前犤(7.59±1.83),(12.63±3.65)mmol/L,(6.33±1.28)%犦明显下降(t=14.496,8.493,4.644,P<0.01)。总有效率分别为94%(33/35),80%(28/35),80%(28/35)。治疗后胆固醇、三酰甘油有明显下降(t=12.400,7
