[目的]探讨外出打工史与肺结核病发病的联系。[方法]采用病例对照研究,从结核病防治机构发现的肺结核人中选取160例25~60岁的涂阳肺结核病人作为病例,每个病例选取2名作为对照。[结果]160名肺结核病人与320名对照者有外出打工史的分别占46.9%、29.4%,二者差异具有统计学意义,OR值为2.4;近3年外出打工史分别占31.9%、17.8%,二者差异有统计学意义,OR值为2.4。采用多因素分析,控制了混杂因素后,外出打工史、近3年外出打工史的OR值仍为2.2、2.0。男性外出打工史、近3年外出打工史与肺结核发病显著性关联,其校正后的OR值分别为2.7、2.4。进一步采用多因素logistic回归模型分析,外出打工的工作场所条件不舒适、每天工作时间超过8h、每周没有休息日的OR值与95%CI分别为4.8(1.7~13.8)、4.9(1.5~16.0)、4.7(1.4~15.4)。[结论]男性外出打工与肺结核发病密切相联系,外出打工工作场所条件差,工作时间较长、每周没有休息日是农民外出打工发生肺结核的重要危险因素;女性外出打工与肺结核有无联系尚需进一步研究。
[Objective] To explore the relationship between the history of migrant workers and the incidence of pulmonary tuberculosis. [Methods] A case-control study was conducted in which 160 cases of smear-positive pulmonary tuberculosis patients aged 25 to 60 years were selected from the tuberculosis patients found in the TB prevention and treatment institutions. Two cases were selected as control in each case. [Results] There were statistically significant differences between the 160 TB patients and 320 controls (46.9% and 29.4% respectively), with a odds ratio of 2.4 and 31.9% and 17.8 respectively %, The difference between the two was statistically significant, OR value of 2.4. Multivariate analysis, control of the confounding factors, go out to work history, the history of migrant workers in the past 3 years the OR was still 2.2,2.0. Men migrate to work history, the past three years the history of migrant workers and pulmonary tuberculosis incidence was significantly associated with the corrected OR values were 2.7,2.4. Further analysis using multivariate logistic regression showed that the working conditions of going out to work were not comfortable and the daily work time was more than 8 hours. The OR and 95% CI of no rest day each week were 4.8 (1.7 ~ 13.8), 4.9 (1.5 ~ 16.0 ), 4.7 (1.4 ~ 15.4). [Conclusion] Male migrant workers are closely related to the incidence of pulmonary tuberculosis. The poor working conditions of working migrant workers, longer working hours and no weekly rest days are the important risk factors for peasants to go out to work for tuberculosis. Is there any connection between female migrant workers and pulmonary tuberculosis? Need further study
