目的调查社区动脉粥样硬化高危人群中阿司匹林抵抗(AR)或半抵抗(ASR)的发生率及其流行病学特征,并探讨其与危险因素的相关性。方法筛选200例动脉粥样硬化高危患者服用阿司匹林(100mg/d)至少7天以上,用二磷酸腺苷(ADP)和花生四烯酸(AA)作诱导剂测定其前后血小板聚集功能变化及血清血栓烷B2(TXB2)水平测定。结果200例动脉粥样硬化高危人群中AR发生率为4.5%,ASR者占20.7%。血清TXB2,AA、ADP诱导的血小板聚集率与健康对照组相比有显著统计学差异(P<0.01);血清TXB2与血小板聚集率有较好的相关性(r=0.871)。结论社区动脉粥样硬化高危人群服用阿司匹林后部分产生AR或ASR;AR或ASR人群发生冠心病事件的风险高于阿司匹林敏感(AS)人群;检测AA、ADP诱导的血小板聚集率,血清TXB2可作为动脉粥样硬化高危人群发生AR或ASR的评价指标。
Objective To investigate the prevalence and epidemiological characteristics of aspirin resistance (AR) or semi-resistance (ASR) in community-high risk population of atherosclerosis and to explore its association with risk factors. Methods A total of 200 high-risk patients with atherosclerosis were screened for aspirin (100 mg / d) for at least 7 days. The changes of platelet aggregation and serum levels of ADP and arachidonic acid (AA) Thromboxane B2 (TXB2) levels were measured. Results The incidence of AR in 200 high-risk atherosclerotic patients was 4.5% and ASR was 20.7%. Serum TXB2, AA, ADP-induced platelet aggregation rate compared with the healthy control group was statistically significant (P <0.01); serum TXB2 and platelet aggregation rate has a good correlation (r = 0.871). CONCLUSIONS: AR or ASR are partially produced in aspirin-exposed high-risk groups at community risk. The risk of coronary heart disease in AR or ASR populations is higher than that in aspirin-sensitive (AS) populations. The levels of platelet aggregation induced by AA and ADP are detected, Evaluation index of AR or ASR in high-risk population of atherosclerosis.
