目的观察紫癜性肾炎(HSPN)患儿尿TNF-α的变化,并探讨其在HSPN发展过程中的作用及其检测意义。方法选取40例HSPN患儿和40例健康对照组儿童,留取24h尿,采用放射免疫法测定其尿TNF-α水平,尿分析仪测定其24h尿蛋白定量,采用SPSS13.0软件进行统计学分析。结果HSPN组与健康对照组患儿尿TNF-α水平比较有显著性差异(P<0.05),且尿TNF-α水平与24h尿蛋白定量呈正相关(r=0.696P<0.05)。结论TNF-α参与了HSPN的发病及迁延进展,且与临床肾脏病变程度密切相关,检测患儿尿TNF-α水平对判断HSPN患儿病情及估计预后具有一定的参考价值。
Objective To observe the changes of urinary TNF-α in children with purpura nephritis (HSPN) and to explore its role in the development of HSPN and its significance. Methods Forty children with HSPN and 40 healthy controls were enrolled in this study. 24-h urinary excretion was collected. Urinary TNF-α levels were measured by radioimmunoassay. Urinary protein was measured by urinalysis. The data were analyzed by SPSS13.0 software analysis. Results The level of urinary TNF-α in HSPN group was significantly lower than that in healthy control group (P <0.05). Urine TNF-α level was positively correlated with urinary protein excretion in 24h (r = 0.696P <0.05). Conclusions TNF-α is involved in the pathogenesis and progression of HSPN, and is closely related to the degree of clinical nephropathy. Detecting the level of urinary TNF-α in children with HSPN has some reference value for judging the prognosis and prognosis of children with HSPN.
