【目的】研究白血病患者化疗前后血清中内毒素浓度,加用白细胞介素-11(IL-11)降低白血病患者化疗后菌血症的发生。【方法】①采用放射免疫法检测40例白血病患者和10例良性血液病患者血清中内毒素浓度;②对发热病人常规抽取静脉血送血细菌培养。【结果】①对照组、IL-11组、普通化疗组血清中内毒素浓度有显著性差异(P<0.05),IL-11组内毒素浓度明显低于普通化疗组(P<0.05);②血细菌培养结果显示对照组(16%)、IL-11组(11%)血细菌培养阳性率明显低于普通化疗组(38%),血清内毒素浓度的升高可促使菌血症发生(P<0.05);【结论】①IL-11组化疗后血内毒素浓度低于普通化疗组化疗后浓度。可能与IL-11抑制炎症因子分泌,降低内毒素血症和感染发生率有关。②IL-11组白血病病人血培养阳性率小于普通化疗组白血病病人。可能与IL-11保护化疗病人黏膜组织,减少菌血症,降低感染发生率有关。
【Objective】 To study the concentration of endotoxin in serum of patients with leukemia before and after chemotherapy and to increase the incidence of bacteremia after chemotherapy by leukemia patients by using interleukin-11 (IL-11). 【Methods】 (1) Radioimmunoassay was used to detect the serum concentration of endotoxin in 40 patients with leukemia and 10 patients with benign hematological diseases. (2) Bacterial cultures of venous blood were routinely drawn from febrile patients. 【Results】 ① The concentration of endotoxin in control group, IL-11 group and general chemotherapy group were significantly different (P <0.05). The concentration of endotoxin in IL-11 group was significantly lower than that of general chemotherapy group (P <0.05); The results of blood culture showed that the positive rate of blood culture in the control group (16%) and IL-11 group (11%) was significantly lower than that in the normal chemotherapy group (38%). The increase of serum endotoxin concentration could promote the occurrence of bacteremia P <0.05). 【Conclusion】 (1) The concentration of endotoxin in IL-11 group after chemotherapy is lower than that in normal chemotherapy group. It may be related to IL-11 inhibiting the secretion of inflammatory cytokines, reducing the incidence of endotoxemia and infection. ② The positive rate of blood culture in IL-11 group leukemia patients was less than that of the normal chemotherapy group leukemia patients. May be associated with the protection of IL-11 chemotherapy in patients with mucosal tissue to reduce bacteremia and reduce the incidence of infection.
