探讨α-硫辛酸(alpha lipoic acid,ALA)治疗糖尿病周围神经病变(diabetic peripheral neuropathy,DPN)对氧化-抗氧化平衡、内质网稳态的影响。选择2010年1月至2014年9月北京市大兴区人民医院DPN患者100例,每组50例。采用SPSS 19.0生成随机数字表,将100例患者按随机数字表分为对照组和治疗组,治疗组患者给予ALA 600mg,1次/d,连续3周。对照组患者给予等量的氯化钠注射液。同选健康组50例,观察DPN患者血清氧化-抗氧化水平变化。结果显示,与健康组相比,DPN患者血清iNOS、MDA、NO和ROS氧化水平增高(P<0.01),而GSH-PX、SOD、T-AOC、维生素C和维生素E抗氧化水平降低(P<0.01),经ALA治疗后氧化水平降低、抗氧化水平升高(P<0.01),恢复平衡状态,而对照组没有这种变化。DPN患者肝脏内质网应激标志物IRE1α蛋白增高(P<0.01),经ALA治疗后降低(P<0.01)。与对照组相比,ALA治疗DPN总有效率增高(P<0.01),能改善患者肢体感觉、麻木、灼热、疼痛、MCV和NCV等临床症状。DPN患者导致机体氧化平衡异常和内质网应激,ALA能改善DPN患者氧化-抗氧化平衡状态和内质网稳态,有效缓解临床病征,治疗效果显著,值得临床进一步应用。
To investigate the effects of alpha lipoic acid (ALA) on oxidative-anti-oxidative balance and endoplasmic reticulum homeostasis in diabetic peripheral neuropathy (DPN). Select January 2010 to September 2014 in Beijing Daxing District People’s Hospital DPN patients 100 cases, 50 cases in each group. A random number table was generated by SPSS 19.0. 100 patients were divided into control group and treatment group according to random number table. Patients in treatment group were given ALA 600 mg once a day for 3 weeks. Patients in the control group were given the same amount of sodium chloride injection. Fifty healthy volunteers were selected to observe the changes of serum oxidative-anti-oxidation in patients with DPN. The results showed that the serum levels of iNOS, MDA, NO and ROS in DPN patients were significantly increased (P <0.01) and the levels of GSH-PX, SOD, T-AOC, vitamin C and vitamin E were lower <0.01). After treatment with ALA, the level of oxidation decreased and the level of antioxidation increased (P <0.01), and the state of equilibrium was restored, while the control group did not. The protein level of endoplasmic reticulum stress index (IRE1α) in patients with DPN increased (P <0.01), and decreased after treatment with ALA (P <0.01). Compared with the control group, ALA treatment of DPN increased the total effective rate (P <0.01), can improve the patient’s limb sensation, numbness, burning, pain, MCV and NCV and other clinical symptoms. DPN patients lead to abnormal oxidative balance and endoplasmic reticulum stress. ALA can improve oxidation-antioxidant balance and endoplasmic reticulum homeostasis in DPN patients, and effectively relieve clinical symptoms. The therapeutic effect is significant and worth further clinical application.
