目的:研究羟丁酸钠对学龄前儿童静脉滴注氯胺酮药代动力学的影响。方法:16例3.5~6岁行择期手术的学龄前儿童随机分为两组:Ⅰ组为氯胺酮组,Ⅱ组为氯胺酮+羟丁酸钠,两组均于气管插管后静脉滴注0.05%氯胺酮3mg·kg~(-1)·h~(-1),用反向高效液相色谱法测定氯胺酮血药浓度,采用中国药理学会编辑的3P_(87)软件计算药代动力学参数。结果:两组房室模型均拟合为二房室开放模型。药代动力学参数;Ⅰ组T_(1/2)α=0.15±0.04h,T_(1/2)β=2.84±0.44h,Cl=0.34±0.06L·kg~(-1)·h~(-1),Vd=0.66±0.29L·kg~(-1)·h~(-1);Ⅱ组T_(1/2)α=0.15±0.04h,T_(1/2)β=2.48±0.45h,Cl=0.43±0.09L·kg~(-1)·h~(-1),Vd=0.64±0.12L·kg~(-1)。结论:羟丁酸钠使氯胺酮清除半衰期缩短,清除率增加。
Objective: To study the effect of sodium oxybate on the pharmacokinetics of ketamine in preschool children. Methods: Totally 16 preschoolers aged 3.5 to 6 undergoing elective surgery were randomly divided into two groups: group Ⅰ was ketamine, group Ⅱ was ketamine + sodium oxybate, and both groups were given intravenous infusion of 0.05% Ketamine 3 mg · kg -1 · h -1. The plasma concentrations of ketamine were determined by reverse-phase high performance liquid chromatography (HPLC). Pharmacokinetic parameters were calculated using 3P_ (87) software edited by the Chinese Pharmacological Society. Results: The two atrioventricular models were fitted as two-compartment open model. The pharmacokinetic parameters of group Ⅰ were as follows: T 1/2 (1/2) α = 0.15 ± 0.04 h, T 1/2 (1/2) β = 2.84 ± 0.44 h and Cl = 0.34 ± 0.06 L · kg -1 (-1), Vd = 0.66 ± 0.29 L · kg -1 · h -1; T 1/2 (1/2) α = 0.15 ± 0.04 h and T 1/2 (1/2) β = 2.48 ± 0.45h, Cl = 0.43 ± 0.09L · kg -1 · h -1, Vd = 0.64 ± 0.12L · kg -1. Conclusion: Sodium oxybate shortens the elimination half-life of ketamine and increases the clearance rate.
