大柴胡汤乃《伤寒论》为少阳未解,复入阳明而设。后世皆有发挥,广泛应用于内、外、妇、儿诸科。郭老师行医六十余年,每用大柴胡汤化裁,治疗外感热病或内伤杂病。以收良效。试举验案,略谈体会。一、眩晕周某,男,21岁,农民。1979年5月2日初诊。头晕目眩,耳鸣二年。曾辗转数院治疗不效。平素脑转耳鸣,食欲不振,不定期眩晕发作,伴听力减退。近三日来,胸胁满闷,口苦咽干,大便燥结,小便短赤,舌质红、苔黄腻,脉弦大。乃热留少阳,浊结阳明为患。治以大柴胡汤化裁:柴胡12克枳实12克黄芩9克大黄12克赤芍9克半夏9克菊花10克防风9克芒硝12克蔓荆子9克木通5克甘草3克二剂,水煎服。药后便通、溲清,眩晕大减,胸胁舒畅,舌质略红,脉眩。阳明燥热已除,少阳热邪已减,故前方去大黄、芒硝、防风、菊花、加麦冬9克,泽泻9克,枣仁10克,远志9克。意在益胃泻肝,安神定志。共服五剂而愈。二、妊娠恶阻陈某,女,26岁,农民。
