病历摘要女性,35岁,农民。该患于1988年9月2日晨6时左右,自服麻黄素7片(总量175mg)后出现心悸、出汗、颤抖。9时感到剧烈头痛,同时伴有呕吐。11时出现失语、右侧半身瘫痪急诊入院。既往无高血压病史。查体:体温36℃,脉搏60次/分,呼吸18次/分,血压160/100mmHg。神志清,运动性失语,双瞳孔等大,光反应存在,右侧鼻唇沟变浅,口角偏向左侧,伸舌左偏。颈软,心肺无异常所见。肝脾不大。右侧半身瘫痪,肌力0级。右 Hoffmann(+),右Babinski(+),左侧肢体活动正常。无其它病理反射。辅助检查:入院后6小时腰穿:为均匀血性脑脊液。空腹血糖5.8mmol/L,胆固醇4.3mmol/L,甘油三脂1.3mmol/L,β-脂蛋白250mg%,血钾4.7mmol/L,钠134mmol/L,氯化物100mmol/L,尿素氮
