目的:探讨服用阿伦膦酸钠复合钙剂对骨质疏松症的4年疗效。方法:60例原发性骨质疏松症患者口服阿仑膦酸钠剂量每周70mg,同时服用乐力每天2g(补充钙剂和维生素D)。分别测定治疗开始前、治疗后6个月、2和4年,腰椎、股骨颈和全髋骨密度,血清碱性磷酸酶(ALP)和血清Ⅰ型胶原交联羧基末端肽(CTX)。结果:与治疗前相比,治疗后6个月、治疗后2年和治疗后4年,腰椎、股骨颈和全髋部位骨密度均显著提高(P<0.01);与治疗6个月比较,治疗后2年和治疗后4年,各部位骨密度仍继续增加均有显著提高(P<0.01);与治疗后2年相比,治疗4年后腰椎骨密度仍继续增加,有显著提高(P<0.05),而股骨颈和全髋部位骨密度基本维持不变。治疗6个月后,ALP和CTX均较治疗前显著下降(P均<0.01),治疗2和4年后,这两种骨转换指标仍维持较低水平,但无进一步显著下降。结论:与原发性骨质疏松症患者治疗前相比,阿仑膦酸钠服用后6个月,2年和4年能够明显降低骨转换生化指标,增加男性原发性骨质疏松症患者的骨密度,且服用4年内安全性良好。
Objective: To investigate the 4-year effect of taking alendronate complex calcium on osteoporosis. Methods: Sixty patients with primary osteoporosis received oral alendronate at a dose of 70 mg weekly, while taking music 2 g daily (supplemental calcium and vitamin D). The bone mineral density (BMD), serum alkaline phosphatase (ALP) and serum type Ⅰ collagen cross-linked carboxy terminal peptide (CTX) of lumbar, femoral neck and total hip were measured before treatment, 6 months, 2 and 4 months after treatment. Results: Compared with those before treatment, the BMD of lumbar, femoral neck and total hip were significantly increased at 6 months, 2 years and 4 years after treatment (P <0.01). Compared with 6 months after treatment, After 2 years of treatment and 4 years after treatment, the BMD of each part still increased significantly (P <0.01). Compared with 2 years after treatment, BMD of lumbar vertebrae continued to increase after 4 years of treatment, with a significant increase ( P <0.05), while the femoral neck and total hip bone mineral density remained unchanged. After 6 months of treatment, both ALP and CTX were significantly lower than those before treatment (all P <0.01). After 2 and 4 years of treatment, the two indicators of bone turnover remained low, but no further decline. CONCLUSIONS: Compared with those before treatment in patients with primary osteoporosis, alendronate at 6 months, 2 years and 4 years after treatment significantly reduced the biochemical markers of bone turnover and increased the incidence of osteoporosis in men Of bone mineral density, and taking good safety within 4 years.
