在肺弥散功能测试中,探讨从甲烷(CH_4)替换氦(He)作为示踪气体对有效肺泡容量 V_A 测定的影响。采用一次呼吸法,分别通过氦分析仪及快速红外分析仪,对30例不同肺部疾病患者进行 He 稀释法和 CH_4稀释法自身对比观察。结果表明,有效肺泡容量结果分别为5.04±1.35(V_ACH_4)和4.88±1.24L(V_AHe),V_ACH_4值略大,但相关系数仍达0.7679。其 CO 弥散量和弥散常数也相关良好。结论认为,He 和 CH_4虽溶解度及分子量有差别,但在肺泡容量及弥散量测定上无影响,也不受气流阻塞及气体分布不均的影响。
In the lung diffusion test, the effect of methane (CH_4) replacement of helium (He) as tracer gas on the determination of effective alveolar volume V_A was investigated. One breath method was used to compare the He dilution method and CH_4 dilution method in 30 patients with different lung diseases by helium analyzer and rapid infrared analyzer respectively. The results showed that the effective alveolar volume results were 5.04 ± 1.35 (V_ACH_4) and 4.88 ± 1.24L (V_AHe), V_ACH_4 slightly larger, but the correlation coefficient still reached 0.7679. The CO dispersion and dispersion constant are also well correlated. The conclusion is that He and CH_4, although different in solubility and molecular weight, have no effect on the determination of alveolar volume and dispersion, and are not affected by the obstruction of airflow and the uneven distribution of gas.
