目的:研究评价硫酸镁联合硝苯地平治疗妊娠期高血压疾病(HDCP)的效果,为临床治疗提供参考。方法:随机抽取在海南医学院附属医院产科治疗的90例中重度HDCP患者,均分为观察组及对照组。所有患者给予解痉基本治疗和25%硫酸镁静脉滴注,总量约22 g/d。观察组另外给予硝苯地平片口服治疗。随访观察两组患者疗效、临床检测指标、血液流变学影响及妊娠并发症。结果:观察组临床总有效率为95.6%,高于对照组(82.2%),差异有统计学意义(P<0.05)。观察组血压控制及临床检测指标改善优于对照组,差异均有统计学意义(P<0.05)。治疗后5天,观察组全血黏度和全血还原黏度各项指标均低于对照组,差异均有统计学意义(P<0.05)。观察组治疗后各项妊娠并发症不超过3例,明显少于对照组,差异有统计学意义(P<0.05)。结论:使用硝苯地平与硫酸镁联合治疗中重度HDCP疗效显著,可改善患者肾功能及血液流变学指标,并发症发生率低,安全性高。
Objective: To evaluate the effect of magnesium sulfate combined with nifedipine in the treatment of gestational hypertension (HDCP) and provide reference for clinical treatment. Methods: Ninety moderate-to-severe HDCP patients were randomly selected from obstetrics and gynecology hospital of Hainan Medical College Affiliated Hospital, and divided into observation group and control group. All patients were given antispasmodic basic therapy and 25% magnesium sulfate intravenous infusion, the total amount of about 22 g / d. The observation group was additionally given nifedipine tablets orally. Follow-up observation of two groups of patients efficacy, clinical testing indicators, hemorheology and pregnancy complications. Results: The total effective rate in observation group was 95.6%, higher than that in control group (82.2%), the difference was statistically significant (P <0.05). The improvement of blood pressure control and clinical examination indexes in the observation group was better than that in the control group, with significant differences (P <0.05). Five days after treatment, the indexes of whole blood viscosity and whole blood reduction viscosity in the observation group were all lower than those in the control group, with significant differences (P <0.05). Observation group after treatment of pregnancy complications no more than 3 cases, significantly less than the control group, the difference was statistically significant (P <0.05). Conclusion: The combination of nifedipine and magnesium sulfate has significant curative effect on moderate and severe HDCP, which can improve the renal function and hemorheology indexes. The complication rate is low and the safety is high.
