目的:观察成人房间隔缺损(ASD)封堵术后右心形态和功能的变化。方法:选择27例继发孔型ASD成功进行封堵的患者,分别于术前、术后3 d、1个月、3个月测量左右室舒张末期内径(LVED、RVED)、右房收缩末期内径(RAD)和右心室射血分数(RVEF);脉冲多普勒测量三尖瓣口舒张期早期峰值流速(E峰)、舒张晚期峰值流速(A峰)、三尖瓣口收缩期反流峰值流速(TR)和肺动脉收缩期峰值流速(PFVP)。结果:封堵术后RVED、RAD、RVEF、E峰、A峰、TR和PFVP减小,LVED增大。结论:房间隔缺损封堵术使右心系统的几何构型改善,右心舒缩功能明显增强。
Objective: To observe the changes of right ventricular morphology and function after atrial septal defect (ASD) occlusion. Methods: Twenty-seven patients with successful ASD were enrolled in this study. Left ventricular end-diastolic diameter (LVED, RVED) and right atrial end-systole were measured at preoperative and postoperative 3 days, 1 month and 3 months respectively. (RAD) and right ventricular ejection fraction (RVEF). Pulse wave Doppler was used to measure the early peak diastolic velocity (E peak), late diastolic peak velocity (A peak), tricuspid regurgitation Peak flow velocity (TR) and pulmonary artery peak systolic velocity (PFVP). Results: RVED, RAD, RVEF, E peak, A peak, TR and PFVP decreased and LVED increased after closure. CONCLUSION: Atrial septal defect occlusion improves the geometry of right ventricular system and enhances right ventricular systolic and diastolic function.
