米非司酮(Ru486)系孕酮受体拮抗剂,米索前列醇(简称米索)具有很强的子宫收缩作用,两者合用可产生良好的抗早孕效果。现遇口服米非司酮致过敏反应1例,报道如下。 1 病例介绍 患者,女,23岁,停经45 d于1997年4月6日来我院妇产科门诊检查。查体:子宫稍大,尿p—hcG阳性。B超诊断:早期宫内妊娠。患者要求药物流产。给米非司酮25mg×6片,bid,po,每次25mg,连续3d,第4天晨8时空腹来我院妇产科病房服米索前列醇0.2mg×3片,观察6h～10h。患者回家后即服米非司酮25mg, 服药30min,恶心,呕吐,腹痛,头晕发热,烦躁,全身瘙痒,出现荨麻疹。急来我院就诊,给异丙嗪25mg和地塞米松10mg,肌肉注射,20min后症状好转,1h后症状基本消退。停止服用米非司酮。患者以往身体健康,月经正常,无特殊病史,近期未服用任何药物。第4天患者来我院复诊。
Mifepristone (Ru486) is a progesterone receptor antagonist, misoprostol (referred to as misoprostol) has a strong uterine contractility, the combination of the two can produce good anti-pregnancy effect. In case of oral mifepristone induced allergic reaction in 1 case, reported as follows. 1 case description The patient, female, 23 years old, menopause 45 d on April 6, 1997 came to our hospital obstetrics and gynecology clinic examination. Physical examination: a little larger uterus, urine p-hcG positive. B-ultrasound diagnosis: early intrauterine pregnancy. Patients require medical abortion. To mifepristone 25mg × 6 tablets, bid, po, each time 25mg, continuous 3d, morning 4 on the 4th morning fasting to obstetrics and gynecology ward misoprostol 0.2mg × 3 tablets observed 6h ~ 10h . After taking home, the patients took mifepristone 25 mg and took medicine for 30 minutes. Nausea and vomiting, abdominal pain, dizziness and fever, irritability, systemic itching and urticaria occurred. Urgent to our hospital, to promethazine 25mg and dexamethasone 10mg, intramuscular injection, 20min after the symptoms improved, 1h after the symptoms subsided. Stop taking mifepristone. In the past, patients had good health, normal menstruation, no special medical history and no recent medication. Day 4 patients to our hospital referral.
