目的探究MRI检查在胎儿中枢神经系统畸形诊断中的应用价值。方法选取我院2014年9月至2016年1月收治的120例孕妇作为研究对象,在胎儿出生后经随访影像结果或尸检结果确诊,有4例胎儿中枢神经系统畸形。分别采取对产前超声(Ultrasound,US)检查(对照组)与磁共振成像(Magnetic Resonance Imaging,MRI)检查(研究组),分析两组检查判断胎儿中枢神经系统畸形结果的正确率及漏诊率。结果超声检查结果显示5例胎儿中枢神经系统畸形,诊断符合率为99.17%,误诊率为0.86%;磁共振检查结果中4例胎儿中枢神经系统畸形均检出,诊断符合率为100.00%,误诊率为0.00%。两组检查结果比较,MRI检查的诊断符合率比超声检查更高,但差异无统计学意义(P>0.05);MRI与超声检查图像质量相比,分辨率更高、视野更大、组织特异性更高且成像平面更标准,但脉络丛室管膜显示效果、血流显示效果不如超声检查,两者各具优势。结论 MRI检查在诊断胎儿中枢神经系统畸形中诊断符合率高,误诊率低,具有较高的临床应用价值,而超声检查也存在一定的优势,对于脉络丛室管膜及血流的显示效果更佳,如果联合应用超声检查,效果更优。
Objective To explore the value of MRI in the diagnosis of fetal central nervous system deformity. Methods A total of 120 pregnant women who were admitted to our hospital from September 2014 to January 2016 were selected as the study subjects. After the birth of the fetus, follow-up imaging results or autopsy findings were confirmed. There were 4 cases of fetal central nervous system deformity. Ultrasound (US) and Magnetic Resonance Imaging (MRI) were used respectively to study the accuracy and misdiagnosis rate of fetal central nervous system deformity . Results Ultrasound examination revealed 5 fetuses with fetal central nervous system deformity. The diagnostic coincidence rate was 99.17% and the misdiagnosis rate was 0.86%. Four cases of fetal central nervous system deformity were detected in magnetic resonance imaging. The diagnostic coincidence rate was 100.00%, misdiagnosis The rate is 0.00%. The diagnostic accuracy of MRI was higher than that of ultrasound in the two groups, but the difference was not statistically significant (P> 0.05). Compared with the quality of ultrasound in MRI, MRI had higher resolution, larger field of view and more tissue specificity Higher sex and imaging plane more standard, but the choroid plexus ependymal display, blood flow display as ultrasound, both have their own advantages. Conclusion MRI examination in the diagnosis of fetal central nervous system malformations with high coincidence rate, low misdiagnosis rate, has a high clinical value, and ultrasound there are some advantages, for the choroid plexus and blood flow display more Good, if the joint application of ultrasound, the better.
