作者为预测特发性血小板减少性紫癜(ITP)患者脾切除的效果,报道19例ITP,包括男性4例,女性15例,年龄12～66岁,中数36岁。术前作骨髓巨核细胞数和~(51)Cr血小板转换率等项测定。摘脾选择的标准:1.血小板数<4万;2.无贫血及白细胞减少(缺铁性贫血例外);3.排除脾机能亢进症;4.除外系统性红斑狼疮;5.除外骨髓再生低下;6.排除骨髓浸润性病变;7.排除DIC;8.排除药物性血小板减少;9.肾上腺皮质激素治疗6个月无改善;10.骨髓巨核细胞数增加(包括正常范围)。
To predict the efficacy of splenectomy in patients with idiopathic thrombocytopenic purpura (ITP), 19 patients with ITP were reported, including 4 males and 15 females, aged 12-66 years and median age 36 years. Preoperative bone marrow megakaryocytes and ~ (51) Cr platelet conversion rate and other items. Pick the spleen selection criteria: 1. Platelet count <40,000; 2. No anemia and leukopenia (iron deficiency anemia exception); 3. Exclusion of hyperactivity; 4. Exclusion of systemic lupus erythematosus; 5. Excluding bone marrow regeneration Low; 6. Excluded myelodysplastic lesions; 7. Exclusion of DIC; 8. Exclusion of drug-induced thrombocytopenia; 9. Adrenal cortex hormone treatment 6 months without improvement; 10. Marrow megakaryocytes increased (including the normal range).
