目的对1例由IgG-Di!a抗体引起的新生儿溶血病进行分析探讨。方法检测患儿及父母的ABO、Rh血型及Diego血型;对患儿进行新生儿溶血病三项实验,并检测患儿母亲血清与患儿红细胞是否反应;对患儿母亲血清及患儿红细胞放散液进行不规则抗体筛查和鉴定,同时测定不规则抗体的效价。结果患儿血型为O型C-c+D+E+e-、Di(a+),患儿母亲为O型C-c+D+E+e-、Di(a-),患儿父亲为O型C-c+D+E+e+、Di(a+);患儿新生儿溶血病检测直抗、游离、释放试验、患儿母亲血清与患儿红细胞反应均阳性,证实患儿患有新生儿溶血病;患儿母亲血清及患儿红细胞放散液中均检出IgG-Di!a抗体,患儿母亲血清中IgG-Di!a抗体效价为16。结论中国人群Di(a+)抗原的频率低,在选择对新生儿溶血病检测实验中所使用的ABO细胞、谱细胞以及不规则抗体筛查使用的筛查细胞时,应注意IgG-Di!a等低频抗体是否会存在漏检。
Objective To analyze and analyze 1 hemolytic disease caused by IgG-Di! A antibody in neonates. Methods The ABO, Rh blood group and Diego blood group of children and their parents were detected. Three experiments were performed on neonates with hemolytic disease of neonates and their mothers serum and erythrocytes of children were tested. The mothers’ serum and erythrocytes of children were released Liquid irregular antibody screening and identification, and determination of irregular antibody titer. Results The blood type of children was O-type C-C + D + E + e-, Di (a +), the mother of the children was O-type C-c + D + E + e-, Di O-type C-C + D + E + e +, Di (a +); children with hemolytic disease detection of neonatal direct resistance, free, release test, children’s mother’s serum and children with red blood cell reaction were positive, Hemolytic disease; IgG-Di! A antibody was detected in the mother’s serum of the children and the erythrocyte discharge of the children. The titer of the IgG-Di! A antibody in the serum of the mother’s child was 16. Conclusions The frequency of Di (a +) antigen in Chinese population is low. In choosing screening cells for ABO cells, spectrum cells and irregular antibody screening for neonatal hemolytic disease testing, we should pay attention to IgG-Di! A Low-frequency antibodies such as the existence of undetected.
