与社区组织合作开展MSM的HIV抗体检测和随访管理效果分析

目的探讨疾病预防控制中心与社区组织(CBO)合作,开展对男男性行为人群(MSM)艾滋病病毒(HIV)抗体检测和随访管理的效果。方法 2011-2013年,在上海市虹口区建立疾病预防控制中心、社区卫生服务中心和社区组织合作网络,针对MSM开展HIV抗体动员检测、阳性发现和随访管理服务,收集HIV检测和随访管理的数据,并与同期疾病预防控制中心艾滋病自愿咨询检测门诊(VCT)以及医疗机构来源的数据进行比较。结果三年间与社区组织合作共动员2190人次MSM接受HIV检测,累计发现报告HIV阳性者77例,占虹口区累计新发现MSM HIV阳性者人数的37.56%(77/205),累计管理MSM HIV阳性者54例,占虹口区累计新管理MSM HIV阳性者人数的40.00%(54/135)。社区组织动员新发现的HIV感染者和艾滋病病人(PLWHA)数、管理数、随访率和首次CD+4T淋巴细胞均值均高于疾病预防控制中心VCT门诊及医疗机构途径发现的PLWHA。结论与社区组织合作能有效提高MSM PLWHA发现和随访管理质量。 Objective To explore the effect of CDC collaborating with community organizations (CBOs) on the detection and follow-up management of HIV antibody in men who have sex with men (MSM). Methods In 2011-2013, a CDC, community health service center and community organization cooperation network was established in Hongkou District of Shanghai. Data on HIV detection and follow-up management were collected for HIV detection, positive detection and follow-up management services for MSM And compared with data from the Centers for Disease Control and Prevention AIDS Voluntary Counseling and Testing Clinic (VCT) and from medical institutions in the same period. Results A total of 2190 HIV-positive MSM patients were mobilized in collaboration with community-based organizations in three years. A total of 77 HIV-positive cases were reported, accounting for 37.56% (77/205) of the total newly diagnosed MSM HIV positive cases in Hongkou District. MSM HIV was cumulatively managed 54 cases accounted for 40.00% (54/135) of the total newly-diagnosed MSM HIV-positive persons in Hongkou District. Community-based mobilization of new HIV-infected and AIDS patients (PLWHA), management numbers, follow-up rates and first-time CD + 4T lymphocyte mean were higher than PLWHA found by the Centers for Disease Control and Prevention VCT Clinic and Medical Institutions pathway. Conclusion Collaboration with community organizations can effectively improve the quality of MSM PLWHA discovery and follow-up management.
