本文分析了51例脑干肿瘤的临床表现,其中儿童24例(47.1％)。病变累及桥脑者42例,仅侵犯中脑8例,累及延髓1例。中脑肿瘤5例以头痛,呕吐为首发症状,桥脑延髓肿瘤有时以反复呕吐起病。入院时体征以颅神经麻痹合并锥体束征最多见,其中面神经麻痹35例(68.6％),三叉神经32例(62.7％),外展神经29例(56.9％),舌咽与迷走神经28例(54.9％),锥体束征32例(62.7％)。病程中有某些症状缓解与复发4例。病理检查14例,计星形细胞瘤11例,少突胶质细胞瘤、髓母细胞瘤与成血管细胞瘤各1例。本文讨论了脑干肿瘤的诊断要点及有关的治疗问题。
This article analyzes the clinical manifestations of 51 cases of brain stem tumors, of which 24 cases of children (47.1%). Lesions involving the bridge of the brain in 42 cases, only infringement of the midbrain in 8 cases, involving the medulla oblongata in 1 case. 5 cases of brain tumor headache, vomiting as the first symptom, pontine pontine tumor sometimes vomiting onset. There were 35 cases (68.6%) with facial paralysis, 32 cases with trigeminal nerve (62.7%), 29 cases with abducens nerve (56.9%), 28 cases with glossopharyngeal and vagus nerves (54.9%), pyramidal signs in 32 cases (62.7%). There are some symptoms in the course of remission and relapse in 4 cases. Pathological examination in 14 cases, including 11 cases of astrocytoma, oligodendroglioma, medulloblastoma and hemangioblastoma in 1 case. This article discusses the diagnosis of brain stem tumor and related treatment issues.
