目的观察分析血液病患者焦虑情绪对免疫功能的影响及如何进行有效的护理。方法使用汉密顿抑郁量表(HRSD)对患者的焦虑程度进行评估,采取4分5级评分法进行评分。通过自填调查问卷进行患者资料的收集。通过静脉血检测观察患者免疫功能情况,包括:免疫学检验技术检测T细胞亚群,Ig A、Ig G、Ig M,血清C3、总T细胞、辅助诱导T细胞、抑制细胞毒T细胞、NK淋巴细胞。加强护理措施,对于焦虑患者应采取心理疏导,了解患者的述求,与患者建立良好的护患关系,这有利于提高患者的治疗依从性,强化治疗效果。同时要对患者家属进行健康讲座的宣教,培训家属护理患者的技巧。在医院和家人的双重关怀下,常常能够帮助患者及早摆脱焦虑状态,重新建立康复的信心。结果本研究发现血液病患者存在焦虑、抑郁情绪障碍。其中存在可能焦虑的患者为54%,肯定焦虑和明显焦虑分别为6%;轻、中度抑郁状态的患者为18%。Ig A、Ig G、Ig M与焦虑情绪呈正相关,差异性具有统计学意义(P<0.05)。结论良好的护理干预,会有效缓解患者不良情绪,提高其免疫功能,提高患者生活质量和治疗效果。
Objective To observe and analyze the influence of anxiety in patients with hematological diseases on immune function and how to carry out effective nursing. Methods Patients were assessed for anxiety using the Hamilton Depression Rating Scale (HRSD) and were scored on a 4: 5 scale. Patient information was collected through self-administered questionnaires. The immune function of the patients was observed by venous blood test, including: T cell subsets, Ig A, Ig G, Ig M, serum C3, total T cells, assistant induced T cells, cytotoxic T cells, NK cells Lymphocytes. Strengthen nursing measures, psychological counseling for patients with anxiety should be to understand the patient’s demands, and patients to establish a good nurse-patient relationship, which is conducive to improving the patient’s compliance with the treatment, and to strengthen the treatment effect. At the same time, we should educate the families of the patients about the health talks and train their families on the skills of nurse patients. Under the double care of the hospital and family members, it is often helpful to help patients get rid of anxiety as early as possible and rebuild the confidence of rehabilitation. Results The study found that patients with blood diseases have anxiety and depression disorders. There were 54% of patients with possible anxiety, 6% of affirmative anxiety and significant anxiety, respectively, and 18% of patients with mild to moderate depression. Ig A, Ig G and Ig M were positively correlated with anxiety, the difference was statistically significant (P <0.05). Conclusion Good nursing intervention can effectively alleviate the adverse emotions of patients, improve their immune function and improve the quality of life and treatment of patients.
