目的:探讨非吸烟女性原发性干燥综合征(pSS)相关间质性肺疾病(ILD)的危险因素。方法:选取2014年1月至2019年11月内蒙古自治区人民医院呼吸与危重症医学科收治的82例非吸烟女性pSS患者，年龄(56.58±13.28)岁，年龄范围为26～84岁。根据高分辨率CT(HRCT)表现分为合并ILD(pSS-ILD)组(n n＝34)及不合并ILD(pSS-N-ILD)组(n n＝48)。比较两组患者的一般资料及实验室化验结果。n 结果:pSS-ILD组患者的发病年龄[(59.76±12.26)岁]显著高于pSS-N-ILD组[(43.95±12.38)岁]，差异有统计学意义(n P0.05)。pSS-ILD组抗核抗体滴度(1 280.00)和C-反应蛋白(4.58 mg/L)明显高于pSS-N-ILD组(640.00，3.20 mg/L)，差异均有统计学意义(n P<0.05)。进一步行logistic回归分析提示，高发病年龄(n OR＝0.889，95%n CI：0.839～0.942，n P<0.001)及抗核抗体滴度升高(n OR＝0.585，95%n CI：0.355～0.963，n P＝0.035)是非吸烟女性pSS患者出现ILD的独立危险因素。n 结论:高发病年龄和抗核抗体滴度升高是非吸烟女性pSS患者发生ILD的独立危险因素，且pSS-ILD的发生与C-反应蛋白水平的升高密切相关，而雷诺现象和消化系统受累也可能预示pSS-ILD的发生。“,”Objective:To investigate the risk factors of primary Sj?gren′s syndrome (pSS) related interstitial lung disease (ILD) in non-smoking women.Methods:A retrospective study was performed on 82 cases of non-smoking women who were admitted to the department of respiratory and critical medicine, Inner Mongolia People′s Hospital from January 2014 to November 2019.According to the High resolution computerized tomography (HRCT) presentation, the patients were divided into the combined ILD(pSS-ILD) group (n n=34) and the uncombined ILD (pSS-N-ILD) group(n n=48). The general information and laboratory test results of the two groups were compared and analyzed.n Results:The age of onset of patients in the pSS-ILD group[(59.76±12.26) years old] was significantly higher than that in the pSS-N-ILD group[(43.95±12.38) years old], and the difference was statistically significant (n P0.05). The titers of antinuclear antibody (1 280.00) and C-reactive protein(4.58 mg/L) in the pSS-ILD group were significantly higher than those in the pSS-N-ILD group[640.00, 3.20 mg/L, (n P<0.05)]. Further logistic regression analysis indicated that the age of high incidence was (n OR=0.889, 95%n CI: 0.839 to 0.942, n P<0.001) and increased antinuclear antibody titer (n OR=0.585, 95%n CI: 0.355 to 0.963, n P=0.035) were independent risk factors for the occurrence of ILD in non-smoking women with pSS.n Conclusion:High age of onset and increased antinuclear antibody titer are independent risk factors for ILD in non-smoking women with pSS patients, and the occurrence of pSS-ILD is closely related to the increase of CRP level.Reynolds phenomenon and digestive system involvement may also predict the occurrence of pSS-ILD.
