目的探讨全自动乳腺容积扫描(ABVS)在乳腺BI-RADS分类的应用价值。方法对159例患者(共179个结节)分别采用二维超声(2DUS)和ABVS进行BI-RADS分类,将2类及3类归为良性;4类及5类归为恶性。以病理结果为标准,比较两种方法诊断乳腺肿块的诊断效能。结果 2DUS诊断乳腺肿块的灵敏度83.0%、特异度77.8%、准确率79.3%、阳性预测值61.1%、阴性预测值91.6%;ABVS的灵敏度96.2%、特异度89.6%、准确率91.6%、阳性预测值79.7%、阴性预测值98.3%。ABVS诊断乳腺恶性肿块的灵敏度、特异度、准确率、阳性预测值及阴性预测值均高于2DUS(P<0.05)。结论 ABVS可以提高二维超声对乳腺肿块BI-RADS分类的准确率和良恶性肿瘤的鉴别诊断。
Objective To investigate the value of automatic breast volume scanning (ABVS) in the classification of breast BI-RADS. Methods A total of 159 patients (179 nodules) were classified by BI-RADS using 2DUS and ABVS respectively. Groups 2 and 3 were classified as benign. Groups 4 and 5 were categorized as malignant. Pathological results as a criterion, the diagnosis of breast cancer by two methods to compare the diagnostic efficacy. Results The sensitivity and specificity of 2DUS in diagnosis of breast masses were 83.0%, 77.8%, 79.3%, 61.1% and 91.6%, respectively. The sensitivity and specificity of ABVS were 96.2%, 89.6% and 91.6%, respectively. Positive predictive value The value of 79.7%, the negative predictive value of 98.3%. The sensitivity, specificity, accuracy, positive predictive value and negative predictive value of ABVS in diagnosing breast malignant tumor were higher than that of 2DUS (P <0.05). Conclusion ABVS can improve the accuracy of two-dimensional ultrasound in the classification of breast masses BI-RADS and the differential diagnosis of benign and malignant tumors.
