目的探讨D2-40标记阳性淋巴管密度(LVD)在非小细胞肺癌(NSCLC)组织中的表达以及与临床病理特征的关系。方法采用免疫组化法检测46例NSCLC组织标本及10例肺良性病变组织标本中D2-40表达,计数D2-40阳性LVD,分析NSCLC周边区LVD与临床病理特征的关系。结果 NSCLC组织中肿瘤周边区LVD明显高于肿瘤中心区和良性病变组织[(11.83±4.15)个vs.(5.34±2.26)个和(4.73±2.62)个](P<0.05)。肿瘤周边区LVD与NSCLC的肿瘤大小、TNM分期及淋巴结转移相关(P<0.01),与患者的性别、年龄、病理类型以及肿瘤分化程度无关(P>0.05);肿瘤中心区LVD与上述临床病理参数均无明显相关性(P>0.05)。结论 D2-40可以特异性识别NSCLC组织中淋巴管内皮细胞;其标记的淋巴管主要位于癌周组织。LVD与NSCLC的肿瘤大小、临床分期及淋巴结转移密切相关,可用来评估肿瘤淋巴转移风险。
Objective To investigate the expression of D2-40 positive lymphatic vessel density (LVD) in non-small cell lung cancer (NSCLC) and its relationship with clinicopathological features. Methods Immunohistochemistry was used to detect the expression of D2-40 in 46 NSCLC specimens and 10 specimens of benign lung lesions. The positive rates of D2-40 positive LVD were counted and the relationship between LVD and clinicopathological features was analyzed. Results The LVD in peripheral region of tumor in NSCLC was significantly higher than that in central tumor and benign lesion [(11.83 ± 4.15) vs (5.34 ± 2.26) and (4.73 ± 2.62), respectively] (P <0.05). There was no correlation between LVD and tumor size, TNM stage and lymph node metastasis (P <0.01), but not with gender, age, pathological type and tumor differentiation (P> 0.05) There was no significant correlation between the parameters (P> 0.05). Conclusion D2-40 can specifically identify lymphatic endothelial cells in NSCLC tissues. The labeled lymphatic vessels mainly located in the peritumoral tissues. LVD and NSCLC tumor size, clinical stage and lymph node metastasis are closely related, can be used to assess the risk of tumor lymphatic metastasis.
