目的应用三维斑点追踪成像技术评价肺动脉高压患者右室心肌收缩功能的改变。方法对37例肺动脉高压患者(PAH)行常规及三维超声心动图检查,得出三尖瓣环收缩期峰值速度(TVSPV)、右室整体峰值纵向应变(GLS)与圆周应变(GCS)。采用Pearson相关分析法分析PAH组GLS与肺动脉收缩压(PASP)、TVSPV的相关性。结果 (1)PAH组右室GLS较对照组减低,差异有统计学意义(P<0.05),而两组间右室的GCS差异无统计学意义(P>0.05);(2)Pearson相关分析表明PAH患者GLS与PASP、TVSPV均具有良好的相关性(r=-0.366、0.421,均P<0.05)。结论肺动脉高压患者右室整体应变有所减低,且纵向应变改变早于圆周方向应变;应用三维斑点追踪成像技术可敏感、准确地检测右心室收缩功能异常,从而为临床早期干预提供依据。
Objective To evaluate the changes of contractile function of right ventricular myocardium in patients with pulmonary hypertension by three-dimensional speckle tracking imaging. Methods Totally 37 patients with pulmonary hypertension (PAH) underwent routine and three-dimensional echocardiography. The tricuspid annulus systolic peak velocity (TVSPV), right ventricular total peak longitudinal strain (GLS) and circumferential strain (GCS) were obtained. Pearson correlation analysis was used to analyze the correlation between GLS and pulmonary artery systolic pressure (PASP) and TVSPV in PAH group. Results (1) The GLS of right ventricle of PAH group was lower than that of the control group (P <0.05), but there was no significant difference of GCS between the two groups (P> 0.05). (2) Pearson correlation analysis Show that GLS in patients with PAH and PASP, TVSPV have a good correlation (r = -0.366,0.421, both P <0.05). Conclusion The global strain of right ventricle in patients with pulmonary hypertension is decreased, and the longitudinal strain change is earlier than that in the circumferential direction. The application of three-dimensional speckle tracking imaging technique can detect right ventricular systolic dysfunction sensitively and accurately, thus providing the basis for early clinical intervention.
