目的观察无创正压通气对急性心源性肺水肿的疗效。方法将43例急性心源性肺水肿患者随机分组,对照组22例,给予强心、利尿及扩张血管药物治疗。治疗组21例,在药物基础上加用无创正压通气。24h后监测两组患者心率、呼吸频率、动脉血氧分压、脑钠肽及血管外肺水,观察无创正压通气对急性心源性肺水肿疗效。结果治疗组患者上述指标均有明显改善,与对照组相比差异显著。结论在规范药物抗心衰治疗基础上无创正压通气能够显著减轻急性心源性肺水肿。
Objective To observe the curative effect of noninvasive positive pressure ventilation on acute cardiogenic pulmonary edema. Methods Forty-three patients with acute cardiogenic pulmonary edema were randomly divided into control group (n = 22) and cardiotomy, diuretic and dilation of blood vessels. Treatment group of 21 cases, based on the drug plus non-invasive positive pressure ventilation. After 24 hours, heart rate, respiration rate, arterial oxygen pressure, brain natriuretic peptide and extravascular lung water were monitored in two groups. The curative effect of noninvasive positive pressure ventilation on acute cardiogenic pulmonary edema was observed. Results The above indicators in the treatment group were significantly improved, compared with the control group significant difference. Conclusion Noninvasive positive pressure ventilation can significantly reduce acute cardiogenic pulmonary edema on the basis of standard anti-heart failure treatment.
