目的:观察对老年骨质疏松性股骨转子间骨折患者采用股骨近端防旋转髓内钉(PFNA)固定术联合唑来膦酸治疗的临床效果。方法:抽取2017年1月至2020年2月平顶山市第一人民医院收治的骨质疏松性股骨转子间骨折患者90例,按照随机数字表法分为对照组与研究组,每组45例。对照组接受PFNA固定术,研究组接受PFNA固定术联合唑来膦酸治疗。观察两组手术前后健侧髋部与腰椎骨密度、术后Harris评分、骨折愈合时间、下地时间及视觉模拟评分法(VAS)评分。结果:研究组术后Harris评分为(89.15±5.13)分,高于对照组的(60.08±5.23)分,n P<0.05。研究组骨折愈合时间、下地时间短于对照组(n P<0.05)。研究组术后VAS评分低于对照组(n P<0.05)。研究组术后患者健侧髋部以及腰椎骨密度优于对照组(n P<0.05)。研究组不良反应发生率为17.8%(8/45),不良反应症状较轻,给予对症治疗后得到缓解。n 结论:对老年骨质疏松性股骨转子间骨折患者采用PFNA固定术联合唑来膦酸治疗能缩短骨折愈合时间,提高髋关节功能、降低患者疼痛感,无严重不良反应发生。“,”Objective:To observe the clinical effects of proximal femoral nail anti-rotation (PFNA) fixation combined with zoledronic acid on elderly patients with osteoporotic intertrochanteric fractures.Methods:A total of 90 patients with osteoporotic intertrochanteric fractures admitted to the First People’s Hospital of Pingdingshan from January 2017 to February 2020 were selected and divided into the control group and study group by random number table method, with 45 cases in each group. The control group received PFNA fixation, while the study group received PFNA fixation combined with zoledronic acid treatment. The preoperative and postoperative bone mineral density of the healthy hip and lumbar spine, postoperative Harris score, fracture healing time, out-of bed time, and visual analogue scale (VAS) score of the two groups were observed.Results:The postoperative Harris score of the study group (89.15±5.13) was higher than that of the control group (60.08±5.23), n P<0.05. The fracture healing time and out-of bed time of the study group were shorter than those of the control group (n P<0.05). The postoperative VAS score of the study group was lower than that of the control group (n P<0.05). The bone mineral density of the healthy hip and lumbar spine of the study group was better than that of the control group (n P<0.05). The incidence of adverse reactions of the study group was 17.8%(8/45), and these adverse reactions were mild, which were alleviated and cured by corresponding treatment.n Conclusions:PFNA fixation combined with zoledronic acid treatment for elderly patients with osteoporotic intertrochanteric fractures can shorten fracture healing time, improve hip joint function, reduce patient pain, without serious adverse reactions.
