目的:探讨锁定钢板内固定结合辨证施治治疗复杂Pilon骨折的临床疗效。方法:68例复杂Pilon骨折均采用锁定钢板内固定并结合中医辨证施治中药内服、外洗治疗。结果:所有病例均获得随访,随访时间1～3年,平均23月,均达骨性愈合,骨折平均愈合时间4.0～7.5月。3例有约8°外翻,5例出现创伤性关节炎症状,其中3例行踝关节融合术。踝关节功能按美国矫形外科足踝协会评分标准进行评分,优良率89.3%。结论:锁定钢板内固定结合中医辨证施治治疗复杂Pilon骨折,能减少并发症发生,促进踝关节功能,是治疗复杂Pilon骨折理想方法之一。
Objective: To investigate the clinical effect of locking plate internal fixation combined with syndrome differentiation in the treatment of complex Pilon fractures. Methods: 68 cases of complex Pilon fractures were treated with internal fixation of locking plate and traditional Chinese medicine syndrome differentiation and internal medicine, external washing treatment. Results: All the cases were followed up for 1-3 years, with an average of 23 months, reaching the bony union and the average healing time of the fracture ranged from 4.0 to 7.5 months. 3 cases had eversion of about 8 ° and 5 cases had traumatic arthritis symptoms, of which 3 cases had ankle arthrodesis. Ankle joint function according to the American Association of Orthopedic Foot and Ankle Association grading standards, excellent rate of 89.3%. Conclusion: The combination of internal fixation with locking plate and traditional Chinese medicine for the treatment of complex Pilon fractures can reduce the incidence of complications and promote the function of the ankle joint. It is one of the ideal methods for the treatment of complex Pilon fractures.
