目的探讨绝经后激素受体阳性的乳腺癌患者血清性激素水平与癌细胞增殖情况之间的关系。方法检测57例雌激素受体阳性的绝经后乳腺癌患者血清性激素6项水平、雌激素受体(ER)、孕激素受体(PR)及增殖细胞核抗原(Ki67)表达状态。将ER和PR阳性的患者根据Ki67的水平分为Ki67低表达组(≤14%)与高表达组(>14%),比较两组间血清激素6项水平的差异。结果在57例ER阳性的患者中,Ki67低表达组睾酮水平高于Ki67高表达组(P=0.01),其余5项两组间的差异无统计学意义(P>0.05)。在40例PR阳性的患者中,Ki67低表达组和高表达组激素6项水平的差异均无统计学意义(P>0.05)。泌乳素在ER阳性和PR阳性的ki67高表达组均较低表达组患者升高,但差异均无统计学意义(P=0.07,P=0.05)。结论在ER阳性的绝经后乳腺癌患者中,Ki67低表达患者的血清睾酮水平较高,睾酮可能有抑制乳腺癌细胞增殖的作用。乳腺癌细胞的增殖情况与血清雌二醇、孕酮水平无关,而可能与泌乳素水平相关。
Objective To investigate the relationship between serum sex hormone levels and cancer cell proliferation in postmenopausal women with hormone receptor-positive breast cancer. Methods Sixty-seven serum estrogen receptor (ER), progesterone receptor (PR) and proliferating cell nuclear antigen (Ki67) were detected in 57 estrogen receptor-positive postmenopausal women with breast cancer. Patients with positive ER and PR were divided into Ki67 low expression group (≤14%) and high expression group (> 14%) according to the level of Ki67, and the differences of serum hormone between the two groups were compared. Results In the 57 cases of ER-positive patients, the level of testosterone in Ki67 low expression group was higher than that in Ki67 high expression group (P = 0.01). There was no significant difference between the other 5 groups (P> 0.05). Among the 40 PR positive patients, there was no significant difference in the 6 levels of Ki67 low expression group and high expression group hormone (P> 0.05). The level of prolactin in both ER positive and PR positive ki67 overexpression groups was higher than that in the low expression group, but the difference was not statistically significant (P = 0.07, P = 0.05). Conclusions In patients with ER-positive postmenopausal breast cancer, the serum testosterone levels in patients with low Ki67 expression are high, and testosterone may have an inhibitory effect on the proliferation of breast cancer cells. Proliferation of breast cancer cells and serum estradiol, progesterone level has nothing to do, and may be related to prolactin levels.
