目的 建立阴茎海绵体动态显像的定量分析方法 ,用于评价血管性阳萎。 方法 用 99m Tc标记 RBC和植酸钙胶体作阴茎海绵体动脉系统、静脉系统显像 ,并在前列腺素 E1 负荷下连续测量阴茎海绵体内放射性变化。观察 5例正常男子和 2 0例阳萎患者阴茎海绵体血液动力学变化。 结果 5例正常男子阴茎动脉系统显像指数 (PIA )为 2 .0 4± 0 .82 (正常值 >0 .6 0 ) ;阴茎静脉系统显像指数 (PIV)为 - 0 .31± 0 .10 (正常值 >- 0 .48)。根据 PIA和 PIV正常值的结果 ,2 0例阳萎患者被分为 3组 :9例 PIA、PIV正常者为非血管性阳萎组 ,其 PIA为 1.96± 0 .71,PIV为 - 0 .32± 0 .0 8;7例 PIA正常、PIV异常者为静脉性阳萎组 ,PIA为 2 .0 1± 0 .76 ,PIV为 - 0 .73± 0 .11;4例 PIA异常 ,PIV正常者或异常者 ,其 PIA为 0 .2 9± 0 .16 ,PIV为- 0 .2 9± 0 .19。 结论 99m Tc标记物阴茎海绵体动态显像对评价血管性阳萎 ,特别是鉴别动脉和静脉系统方面的病理状况是一种安全、无创、客观和有价值的方法。
Objective To establish a quantitative method for the dynamic imaging of penile cavernous tissue to evaluate vascular impotence. Methods 99m Tc labeled RBC and calcium phytate colloid for the penile cavernous system, venous system imaging, and continuously under the prostaglandin E1 radioactivity in the penile cavernous changes. The hemodynamic changes of corpus cavernosum in 5 normal men and 20 impotence patients were observed. Results The imaging index (PIA) of penile arteries in 5 normal men were 2.04 ± 0.82 (normal> 0.6) and penile venous system imaging index (PIV) was -0.31 ± 0. 10 (normal> -0.48). According to the results of PIA and PIV normal values, 20 cases of impotence were divided into 3 groups: 9 cases of PIA, PIV normal non-vascular impotence group, the PIA was 1.96 ± 0.71, PIV was -0. 32 ± 0.08; 7 cases of PIA normal, PIV abnormalities were venous impotence group, PIA was 2.01 ± 0.76, PIV was - 0.73 ± 0.11; 4 cases of PIA abnormalities, PIV Normal or abnormal, the PIA was 0.29 ± 0.16, PIV - 0.29 ± 0.19. Conclusion Dynamic 99m Tc labeling of corpus cavernosus is a safe, noninvasive, objective and valuable method for the evaluation of vascular impotence, especially for the identification of arterial and venous system pathology.
