目的:探讨动态增强MRI对颅内原发性肿瘤的诊断价值。材料与方法:使用1.0TMRI系统(MagnetomImpact,Siemens)SE序列对72例颅内原发性肿瘤进行动态增强扫描,分析各种肿瘤的对比增强率时间(CERT)曲线类型、增强峰值(EP)及最大对比增强率(MCER)。结果:EP在90秒内见于3例血管瘤型脑膜瘤、1例血管母细胞瘤及1例脉络膜乳头状瘤;26例星形细胞瘤仅4例Ⅲ、Ⅳ级的EP在180秒内,而25例脑膜瘤中的13例EP在180秒内,P值<0.01;血管瘤型脑膜瘤、神经鞘瘤、血管母细胞瘤及脉络膜乳头状瘤的EP及MCER明显高于其他肿瘤。结论:动态增强MRI的CERT曲线类型、EP及MCER有助于部分颅内原发性肿瘤的组织学诊断和富血管度评价。
Objective: To investigate the diagnostic value of dynamic contrast-enhanced MRI in primary intracranial tumors. MATERIALS AND METHODS: Seventy-two cases of primary intracranial tumors were dynamically scanned using the 1.0 TMRI system (MagnetomImpact, Siemens) SE sequence. The types of curves of contrast-enhanced rate-time (CERT) (EP) and maximum contrast enhancement (MCER). Results: EP was found in 3 hemangiomas, 1 hemangioblastoma and 1 choroid papilloma in 90 seconds. Only 26 patients with astrocytoma had 4 cases of grade Ⅲ and Ⅳ EP within 180 seconds. While EP in 13 out of 25 meningiomas was within 180 seconds and P value was less than 0.01. EP and MCER of hemangiomas meningioma, schwannoma, hemangioblastoma and choroid papilloma were significantly higher than those of other tumors . Conclusions: The type of CERT curve in dynamic contrast-enhanced MRI, EP and MCER contribute to the histological diagnosis and vascularization of some intracranial primary tumors.
