目的探讨实时组织弹性成像对肝硬化结节的诊断价值。方法对48例肝硬化患者的55个结节(直径≤2cm)进行实时弹性成像,采用应变率比值法定量测定肝硬化结节与周围肝组织的硬度比值。通过受试者工作特征曲线分析结节的良恶性特征。结果通过分析55个病灶的特征,确定应变率比值等于3.18为肝脏良恶性肿瘤的最佳诊断临界值,其诊断敏感度为88.24%,特异度为80.95%,准确性为83.64%,阳性预测值88.20%,阴性预测值81.00%。结论实时超声弹性成像应变率比值法是评价直径≤2cm的肝硬化结节良恶性的一种有效方法。
Objective To investigate the diagnostic value of real-time tissue elastography in the diagnosis of cirrhosis nodules. Methods The real-time elastic imaging of 55 nodules (diameter≤2cm) in 48 cirrhotic patients was performed. The ratio of hardness of cirrhotic nodules to the surrounding liver tissue was quantitatively determined by strain rate ratio method. The characteristics of benign and malignant nodules were analyzed by the receiver operating characteristic curve. Results By analyzing the characteristics of 55 lesions, the ratio of strain rate was equal to 3.18, which was the best diagnostic threshold for benign and malignant liver tumors. The diagnostic sensitivity was 88.24%, the specificity was 80.95%, the accuracy was 83.64%. The positive predictive value 88.20%, negative predictive value 81.00%. Conclusion The real-time ultrasound elastography strain rate ratio method is an effective method to evaluate benign and malignant nodules of cirrhosis with diameter ≤2cm.
