目的探讨飞利浦256层Brilliance iCT低辐射剂量联合迭代重建(IMR)在肺动脉CTA中的应用价值。方法前瞻性搜集临床怀疑肺栓塞行肺动脉CTA检查的62例患者,将其随机分为A、B两组,其中A组32例,采用100 kV,自动mA扫描,图像按滤波反投影方式重建;B组30例,采用100 kV、50 mAs扫描,按IMR重建。由两位高年资放射科医师用5分法主观评价图像质量,利用工作站后处理比较两组间图像质量并记录分值;测量肺动脉主干、左、右肺动脉干及左下、右下肺动脉CT值及SD值,计算其平均值。利用独立样本t检验比较两组间的CT值、SD值、信噪比(SNR)、对比噪声比(CNR)、容积CT剂量指数(CTDI_(vol))、剂量长度乘积(DLP)和有效剂量(ED)的差异。结果两组间图像质量主客观评价差异无统计学意义,两组间患者CTDI_(vol)、DLP和ED方面差异有统计学意义。结论飞利浦256层Brilliance iCT IMR重建技术用于肺动脉CTA检查,其图像质量在满足临床诊断需要的同时,可以有效地降低患者的辐射剂量。
Objective To explore the value of Philips 256-layer Brilliance iCT low-dose radiation combined with iterative reconstruction (IMR) in pulmonary artery CTA. Methods Sixty-two patients with suspected pulmonary embolism undergoing pulmonary CTA were prospectively collected and randomly divided into two groups. Group A (32 cases) was scanned with 100 kV automatic mA and the images were reconstructed by filtered back projection. B group 30 cases, using 100 kV, 50 mAs scan, according to IMR reconstruction. Two high-grade radiologists subjectively evaluated the image quality by a 5-point method. The image quality was recorded by workstation post-processing and the scores were recorded. The CT values of the main pulmonary artery, left and right pulmonary arteries, left and right lower pulmonary arteries were measured And SD value, calculate the average value. The CT value, SD value, signal to noise ratio (SNR), contrast to noise ratio (CNR), volumetric CT dose index (CTDI volt), dose length product (DLP) and effective dose (ED) difference. Results There was no significant difference in subjective and objective evaluation of image quality between the two groups. There was significant difference in CTDI vol, DLP and ED between the two groups. Conclusion The Philips 256-layer Brilliance iCT IMR reconstruction technique is used for CTA examination of pulmonary arteries. Its image quality can effectively reduce the radiation dose of patients while meeting the needs of clinical diagnosis.
