目的初步探讨基质金属蛋白酶2和9(MMP-2,MMP-9)及血管内皮细胞生长因子(vascular endothelial growth factor,VEGF)在颅内囊性动脉瘤表达及意义。方法采用免疫组化方法检测正常脑血管和颅内囊性动脉瘤中MMP-2,MMP-9及VEGF的表达情况;通过透射电镜观察正常脑血管及颅内囊性动脉瘤超微结构变化。结果观察组MMP-2,MMP-9及VEGF表达较对照组显著增高(P<0.05),免疫阳性产物主要分布于动脉瘤壁的中膜层,同时,观察组的内皮细胞变性脱落,胞浆内出现肿胀的线粒体和大小不等的空泡,瘤壁可见形态异常的平滑肌细胞和成纤维细胞,增多的胶原纤维排列紊乱。结论MMP-2,MMP-9及VEGF过度表达可能与血管退行性变、颅内囊性动脉瘤瘤体的形成、扩大、破裂有关。在颅内多发囊性动脉瘤更具重要意义。
Objective To investigate the expression of MMP-2, MMP-9 and vascular endothelial growth factor (VEGF) in intracranial saccular aneurysms. Methods Immunohistochemistry was used to detect the expression of MMP-2, MMP-9 and VEGF in normal cerebrovascular and intracranial saccular aneurysms. The ultrastructural changes of normal cerebrovascular and intracranial saccular aneurysms were observed by transmission electron microscopy. Results The expression of MMP-2, MMP-9 and VEGF in the observation group was significantly higher than that in the control group (P <0.05). The immunopositive products were mainly distributed in the media of the aneurysmal wall. At the same time, the endothelial cells in the observation group degenerated, Within the swollen mitochondria and vacuoles ranging in size, the tumor wall shows abnormal morphology of smooth muscle cells and fibroblasts, an increase of collagen fibers arranged disorderly. Conclusion Overexpression of MMP-2, MMP-9 and VEGF may be related to the degeneration of blood vessel, the formation, expansion and rupture of intracranial cystic aneurysm. In intracranial multiple cystic aneurysm is more important.
