目的探究γ刀分次立体定向放射外科治疗颅内肿瘤的临床疗效。方法 284例颅内肿瘤患者采用γ刀分次立体定向放射外科手段进行治疗,分析治疗结果。结果所有受试者随访1~5年,平均随访(2.55±0.82)年。284例患者中治疗总有效237例(83.45%)。胶质瘤Ⅰ级1年生存率100.00%(26/26);胶质瘤Ⅱ级1年生存率100.00%(35/35);胶质瘤Ⅲ级1年生存率77.50%(31/40);胶质瘤Ⅳ级1年生存率75.00%(24/32);垂体瘤1年生存率100.00%(40/40);脑膜瘤1年生存率82.86%(29/35);松果体区肿瘤1年生存率84.85%(28/33);听神经瘤1年生存率92.59%(25/27);其他肿瘤1年生存率81.25%(13/16)。所有患者经治疗后出现放射性脑水肿75例(26.41%),其中Ⅰ度31例(41.33%)、Ⅱ度25例(33.33%)、Ⅲ度19例(25.33%);放射性颅神经功能障碍16例(5.63%);放射性脑组织坏死13例(4.58%)。结论γ刀分次立体定向放射外科治疗颅内肿瘤的临床疗效较好。
Objective To investigate the clinical efficacy of gamma knife radiofrequency stereotactic radiosurgery in the treatment of intracranial tumors. Methods A total of 284 patients with intracranial tumors were treated by γ knife radiosurgery and stereotactic radiosurgery. The treatment outcome was analyzed. Results All subjects were followed up for 1 to 5 years with a mean follow-up of (2.55 ± 0.82) years. Of the 284 patients, 237 were effective (83.45%). The 1-year survival rate of grade I glioma was 100.00% (26/26). The one-year survival rate of grade II glioma was 100.00% (35/35). The grade 1 glioma survival rate was 77.50% (31/40) ; 1-year survival rate of grade IV glioma was 75.00% (24/32); the one-year survival rate of pituitary adenoma was 100.00% (40/40); the 1-year survival rate of meningioma was 82.86% (29/35) The 1-year survival rate of tumor was 84.85% (28/33). The 1-year survival rate of acoustic neuroma was 92.59% (25/27). The other 1-year survival rate of other tumors was 81.25% (13/16). Radiological brain edema occurred in 75 patients (26.41%) after treatment, including 31 cases (41.33%) of grade Ⅰ, 25 cases (33.33%) of grade Ⅱ and 19 cases (19.3% Ⅲ) of radiological cranial nerve dysfunction Cases (5.63%); radioactive brain tissue necrosis in 13 cases (4.58%). Conclusion Gamma knife radiofrequency grafting for the treatment of intracranial tumors with good clinical efficacy.
