目的:探讨3D-CRT低剂量区V5体积的大小对非小细胞肺癌患者肺功能的影响。方法:收集我院肿瘤科2014年4月~2015年10月收治的46例首次行三维适形放射治疗且顺利完成治疗的非小细胞肺癌患者,对每例患者分别在治疗前以及治疗后的1、3个月进行肺功能检测,分析3D-CRT低剂量区V5体积的大小与肺功能变化之间的关系。结果:放疗后1个月的FEV1%、FVC%值明显高于放疗前且差异均具有显著性。放疗后1个月、3个月的DLCO%值明显低于放疗前,且放疗前的与放疗后3个月的差异具有显著性;V5≦45%、45%55%三组放疗前后的FEV1%值和FVC%值比较差异均不具有显著性。V5≦45%、45%55%三组放疗前后的DLCO%值均呈逐渐下降趋势,且V5>55%组的放疗前后DLCO%值比较差异具有显著性。而V5≦45%组和45%55%时,DLCO%值降低的趋势更加显著。
Objective: To investigate the effect of the volume of V5 in low-dose 3D-CRT area on lung function in patients with non-small cell lung cancer. Methods: Forty-six patients with non-small cell lung cancer who underwent three-dimensional conformal radiotherapy for the first time from April 2014 to October 2015 in our department of oncology were enrolled in this study. Before and after treatment, 1, 3 months for pulmonary function tests, analysis of 3D-CRT low-dose volume V5 volume and lung function changes in the relationship between. Results: At 1 month after radiotherapy, the FEV1% and FVC% values were significantly higher than those before radiotherapy and the differences were significant. The DLCO% values at 1 month and 3 months after radiotherapy were significantly lower than those before radiotherapy, and the difference was significant between pre-radiotherapy and 3 months after radiotherapy; V5 ≦ 45%, 45% 55% of the three groups before and after radiotherapy FEV1% and FVC% values were not significantly different. The values of DLCO before and after radiotherapy were gradually decreased in V5≤45%, 45% 55%, respectively. There was significant difference in DLCO% before and after radiotherapy in V5> 55%. However, there was no significant difference in DLCO% before and after radiotherapy between V5 ≦ 45% and 45% 55%, the DLCO% reduction trend is more pronounced.
