目的比较短期胰岛素泵治疗对不同病程2型糖尿病患者血内脏脂肪素(visfatin,VF)的影响。方法选择2型糖尿病患者100例,根据病程分为观察组40例(初诊病程<1年)和对照组60例(病程1~10年)。两组均使用胰岛素泵持续皮下输注超短效胰岛素(诺和锐)2周。比较两组空腹血糖(FBG)、餐后2h血糖(2hFBG)、糖化血清白蛋白(GA)、空腹内脏脂肪素(VF)、空腹C肽(C-P)等变化。结果两组FPG、2hFPG、GA、C-P、空腹内脏脂肪素差异均有统计学意义(t=3.573,P<0.05);两组治疗前后变化量比较具有统计学意义(t=2.784,P<0.05),VF与FBG、2hFPG及GA成正相关。结论早期胰岛素泵强化治疗能够很好控制血糖水平,对改善β细胞功能及胰岛素抵抗有着重要的作用,而这种作用在1年以上病程的患者中表现相对不明显。
Objective To compare the effects of short-term insulin pump on visfatin (VF) in patients with type 2 diabetes mellitus with different course of disease. Methods 100 cases of type 2 diabetes mellitus were selected. According to the course of disease, 40 cases were divided into two groups: observation group (1 year) and control group (60 cases). Both groups were given continuous subcutaneous insulin infusion with insulin pump (Novo-sharp) for 2 weeks. The fasting blood glucose (FBG), postprandial 2h blood glucose (2hFBG), glycosylated serum albumin (GA), fasting visceral fat (VF) and fasting C peptide (C-P) were compared between the two groups. Results The difference of FPG, 2hFPG, GA, CP and fasting visceral fat between the two groups was statistically significant (t = 3.573, P <0.05). The change of the two groups was statistically significant before and after treatment (t = 2.784, P <0.05 ), VF was positively correlated with FBG, 2hFPG and GA. Conclusions Early insulin pump intensive therapy can control blood glucose level well and play an important role in improving β-cell function and insulin resistance. However, this effect is relatively insignificant in patients with more than one year’s course of disease.
