原发性三叉神经痛的病因目前仍不清楚。正常人脑标本显示桥小脑角三叉神经感觉根与周围血管关系密切。三叉神经感觉根的血管压迫与三叉神经痛有肯定关系,神经血管接触在三叉神经痛病人与无此症同龄人之间有质和量的区别。三叉神经退行性改变,尤其脱髓鞘改变是三叉神经痛的主要病理改变。目前治疗方法主要有药物治疗、经皮射频热凝选择性破坏、甘油用于三叉神经节注射术及颅后窝手术等。显微血管减压术的疗效有待长期观察。
The cause of primary trigeminal neuralgia is still unclear. Normal brain samples showed that the sense of the trigeminal nerve root of the cerebellopontine angle and the surrounding blood vessels are closely related. Trigeminal nerve root compression of blood vessels and trigeminal neuralgia have a positive relationship between neurovascular exposure in patients with trigeminal neuralgia and without the same age and gender differences in quality and quantity. Trigeminal nerve degeneration, especially demyelination is the main pathological changes of trigeminal neuralgia. The current treatment methods are mainly drug treatment, percutaneous radiofrequency thermal coagulation selective destruction of glycerol for trigeminal ganglion injection and posterior fossa surgery. The efficacy of microvascular decompression remains to be seen long term.
