目的 探讨在急性重症胰腺炎时通过动态监测胃粘膜下pH值 (Phi) ,从而早期发现局部组织器官缺氧情况 ,及时采取措施预防器官功能衰竭的发生。方法 对在临床观察期间收住院的 40例急性重症胰腺炎患者在入院后立刻经鼻插入TRIP -NGS导管并测定Phi,以后每隔 12小时测量一次Phi。结果 并发器官功能衰竭的急性重症胰腺炎患者在器官功能衰竭前 1~ 3天Phi即有显著降低 ,而同期测得的全身氧供指标并未发生明显异常。结论 Phi具有操作简便 ,结果可靠等优点 ,对于早期发现局部组织缺氧 ,尽快采用措施预防器官功能衰竭的发生具有重要指导意义。
Objective To investigate the hypoxic condition of local tissues and organs by dynamic monitoring of gastric mucosal pH (Phi) during acute severe pancreatitis and take prompt measures to prevent the occurrence of organ failure. Methods Forty patients with acute severe pancreatitis admitted to the hospital during the clinical observation were enrolled in the TRIP-NGS catheter immediately after admission and Phi was measured. Phi was measured every 12 hours thereafter. Results In patients with acute severe pancreatitis complicated with organ failure, Phi was significantly reduced 1 to 3 days before organ failure, while no significant abnormality was observed in the systemic oxygen supply during the same period. Conclusion Phi has the advantages of simple operation, reliable results and so on. It has important guiding significance for the early detection of local tissue hypoxia as soon as possible to adopt measures to prevent organ failure.
