目的：探讨氨磺必利治疗以阴性症状为主的精神分裂症患者的临床疗效和安全性以及对社会功能的影响。方法将64例以阴性症状为主的精神分裂症患者随机分为两组，研究组口服氨磺必利治疗，对照组口服利培酮治疗，观察12周。治疗前后采用简明精神病量表、阴性症状量表评定临床疗效，副反应量表评定不良反应，住院精神病人社会功能评定量表和社会适应功能评估量表评定社会功能。结果治疗12周末两组简明精神病量表、阴性症状量表评分均较治疗前显著下降（P＜0.01），研究组阴性症状量表的意志缺乏、兴趣／社交缺乏因子分均较对照组下降更显著（ P＜0.01）。研究组总有效率56.3％，对照组为53.1％，两组比较差异无显著性（ P＞0.05）。两组住院精神病人社会功能评定量表和社会适应功能评估量表评分均较治疗前有显著改善（P＜0.01），研究组较对照组改善更显著（P＜0.01）。两组不良反应较轻微，研究组窦性心动过速／ST段改变、血糖异常、体质量增加、震颤、月经紊乱等不良反应发生率均显著低于对照组（ P＜0.05或0.01）。结论氨磺必利治疗以阴性症状为主的精神分裂症疗效显著且与利培酮相当，但氨磺必利在改善患者的意志障碍、兴趣／社交障碍和社会功能方面优于利培酮，安全性更高。“,”Objective To explore amisulpride’s clinical effect ,safety and influences on social function in schizophrenic characterized by negative symptoms (NS) .Methods Sixty‐four NS schizophrenics were ran‐domly assigned to two groups ,research group took orally amisulpride and control group did risperidone for 12 weeks .Before and after treatment clinical effects were assessed with the Brief Psychiatric Rating Scale (BPRS) and Scale for the Assessment of Negative Symptoms (SANS) ,adverse reactions with the Treat‐ment Emergent Symptom Scale (TESS) ,and social function with the Scale of Social Function in Psychosis Inpatients (SSPI) and Social‐Adaptive Functioning Evaluation (SAFE) .Results At the end of the 12th week the BPRS and SANS scores of both groups lowered more significantly comparing with pretreatment (P<0.01) ,so did abulia and social disinterest score of SANS in research than control group (P 0.05) .The SSPI and SAFE scores of both groups improved more signifi‐cantly compared with pretreatment (P<0.01) ,so did those in research than control group (P<0.01) . Adverse reactions of both groups were mild ,the incidence of sinus tachycardia/ST segment change ,patho‐glycemia ,weight gain ,tremor ,menstrual disorder etc .was significantly lower in research than control group (P<0.05 or 0.01) .Conclusion Amisulpride has an evident effect equivalent to risperidone in NS schizophrenic ,but the former does an advantage over the latter in improving abulia ,social disinterest and social function as well as in safety .
