目的:对胃癌组织标本的细胞凋亡和PCNA、P16表达产物进行细胞生物学及临床病理学研究。方法:应用SP免疫组织化学方法及原位TUNEL法。结果:高/中分化癌组,凋亡指数为(10.4±2.1%),低分化癌组和粘液癌组分别为(4.1±2.3%)、(3.5±1.6%)。浸润深度不超过肌层(含肌层)组凋亡指数为(8.6±3.5%)全层浸润组为(4.3±2.1%)。伴淋巴结转移组凋亡指数为(3.7±1.5%),不伴有淋巴结转移组凋亡指数为(9.2±4.1%)。低分化癌组和粘液癌组中PCNA阳性率分别为60%、77.3%,高/中分化癌组为41.7%;浸润深度未超过肌层组(含肌层)中,阳性率为28.6%,全层浸润组阳性率77.2%。在PCNA阳性组凋亡指数为(3.4±1.2%),PCNA阴性组凋亡指数为(7.6±4.1%),二者呈显著性负相关,p<0.05。在高/中分化组P16表达阳性率为66.7%,在低分化组和粘液癌组分别为33.3%、44.4%。结论:胃癌中细胞凋亡与组织的分化程度正相关;与浸润深度和淋巴结转移负相关。PCNA表达与胃癌组织分化程度负相关;与浸润深度、淋巴结转移正相关。联合检测细胞凋亡与PCNA表达有助于对胃癌组织的生物学行为进行评定。p16;与胃癌组织的分化程度正相关。
OBJECTIVE: To study the cell biology and clinicopathological features of apoptosis and expression of PCNA and P16 in gastric cancer tissue samples. Methods: SP immunohistochemical method and in situ TUNEL method. Results: The apoptosis index in high / moderately differentiated cancer group was (10.4 ± 2.1%), that in poorly differentiated carcinoma and mucinous carcinoma was (4.1 ± 2.3%) and (3.5 ± 1.6%) respectively. Apoptosis index (4.3 ± 2.1%) in the full-thickness infiltration group (8.6 ± 3.5%) with infiltration depth no more than muscle layer (including muscle layer). The apoptosis index in lymph node metastasis group was (3.7 ± 1.5%), and the apoptosis index in non-lymph node metastasis group was (9.2 ± 4.1%). The positive rates of PCNA in poorly differentiated carcinoma and mucinous carcinoma were 60%, 77.3%, 41.7% in high / moderately differentiated carcinoma group, respectively. The positive rate was 28.6% in the myometrium group, Full-thickness infiltration of the positive rate of 77.2%. The apoptotic index in PCNA positive group was (3.4 ± 1.2%), and the apoptosis index in PCNA negative group was (7.6 ± 4.1%). There was a significant negative correlation between them (p <0.05). The positive rate of P16 expression in high / moderate differentiation group was 66.7%, 33.3% and 44.4% in poorly differentiated group and mucinous carcinoma group respectively. Conclusion: There is a positive correlation between apoptosis and the degree of differentiation in gastric cancer, and negative correlation with depth of invasion and lymph node metastasis. The expression of PCNA was negatively correlated with the degree of differentiation of gastric cancer, and positively correlated with the depth of invasion and lymph node metastasis. Combined detection of apoptosis and PCNA expression contribute to the assessment of biological behavior of gastric cancer. p16; positive correlation with the degree of differentiation of gastric cancer tissue.
